FEE STRUCTURE

4. A chargeof $5 per daywill be assessed

on all latefees.

5. Pleasenotiff the providerno laterthan8:00a.m.if the child (ren)will not be in carefor the
day.

6.If a child is pickedup laterthanthe time agreedupon,an additionalchargeof $5 per child

shallbe chargedper 15minuteintervalsafterthe agreeduponpick-uptime.

7. An advancenoticeof two weeksis requiredif the child is to be permanently

withdrawnfrom
childcare.Two week'stuition will beacceptedin lieu of a two-weeknotice.Providerwill alsobe
requiredto givetwo weeksnoticeprior to cessionof care,exceptin casesof grossmisconducton
the part of parentof child,

SICK DAYS AND VACATION

There is no chargefor any time taken off by the Child Care Provider for sick days, vacationsor
conferences.In caseof emergencyor illness, the provider will make every attempt to provide
substitutecare.However, parentsshould be preparedwith their own backup.

We take two weeks of vacation eachyear and will give you at leasttwo weeks written notice of
vacation plans.

You may have two weeks of vacation time for which you will pay fifty percent of your weekly
rate. This will hold your spaceon the roster. All paymentsmust be paid prior to vacation.

Your vacation time may be broken up and taken at anytime, as long as we receive two week
notice of two or more consecutivedays that your child will be gone

MEALS

The following meals are servedeach day at no additional charge(check meals that apply)

Should your child arrive later than a meal time, you will be responsiblefor feeding him/her
except for specialoccasionsor conditions requiring special diet. Pleasedo not send any food
with your child. This includes, but is not limited to gum, candy, chips, cookies, cereal, etc.

I am a participant in a Child Nutrition Program and make every attempt to provide enjoyable
nutritious meals for your child. I offer a variety of foods. Your child is encouragedto try new
foods but not forced.

ILLNESS

Should your child becomeill during his/her day here, you will be notified and we will determine
the best courseof action concerningappropriatecare,which may include the child being taken
home.

No child will be acceptedwith a fever in excessof 101,or if he/sheis vomiting, has excessive
diarrhea, or any potentially contagiousdisorder. There are other children to consider and it is
important to think of the well-being of all involved.

Any medication to be given must come in a clearly labeled prescription bottle with dosage
information. Non-prescription medicine, if needed,will also be administeredif provided by
parent.

blanketor otherspecialitems,it is bestthatyour child not bring anythingfrom home.If this
can't be avoided,parentwill acceptresponsibilityfor the items.PLEASEDO NOT sendcandy,
gum,or moneywith your child. It is the policy that whateveris broughtto the progam is to be
sharedwith otherchildrento avoidsquabbles. Theproviderwill not be responsiblefor anyheft,
damage,or lossof toys,books,gamesetc.broughtfrom home.

CHILD ABUSE
We aremandatedby law to reportany signof neglector abuseof the childrenin our careand
will promptlydo so.We areliable asthe child careprofessionfor this responsibilityandcanbe
fined andor jailed for failureto do so.

TOILET LEARNING
Extrachangesof clothingwill be requiredduringthe toilet learningperiod.We arehappyto help
with thetoilet leamingprocessat an appropriateage(24-36)months.It alsohelpsus if the parent
practicesthis processat homeaswell.

ADDRESSAIID PHONE CHANGES

Any andall changesin personaladdressandphonenumberswill be givento theproviderassoon
aspossible.Namesandnumberson the emergency form will be kept cunent.

MEDICAL EMERGENCIES
Note thatyour child will only be releasedto personsotherthantheparentor legalguardianif
their nameappearson your signedEmergencyForm.In caseof an emergency, a phonecall will
sufficeaslong asthe person picking up the child showsidentification.

TRANSPORTATION
for transportingyour child to andfrom the child carehome.

Occasionallywe needto takeour own childrento activitiesthatthey areinvolvedin or we may
takea field trip excursionandwill providetransportation.
lnfantswill be placesin safety-
approvedcar seatswhich will be providedby parents.All childrenwill be requiredto weara seat
belt at all times.We carrya notebookanda duplicatecopyof emergeney formswith a pictureof
eachchild attachedto his/herform. In the eventof an emergencyawayfrom home,your child
will be caredfor andyou will be notified.We do askfor your permissionto travelin a car.
