“RESOLUTION RECOMMENDING THE PROCUREMENT by DIRECT


CONTRACTING of COMPANY X as the INTERNET SERVICE PROVIDER of
CITY/MUNICIPALITY, PROVINCE

WHEREAS, the approved budget to avail the services of an internet service


provider for City/Municipality, Province is ________________ (Php ________), i.e.,
________ (Php ______) for installation and _________ (Php _______) as monthly
payment for ____ years;

WHEREAS, ____________________________, upon submission of the


concerned Office of a Cost Benefit Analysis and Summary of Price Quotations
identified that Company X is the only internet service provider in City/Municipality,
Province that met the required specifications under the herein attached Purchase
Request;

WHEREAS, the End-user recommended to procure the services through Direct


Contracting of Company X, being the only internet service provider in the concerned
Office and for which no suitable substitute can be obtained at a more advantageous
terms to the government, pursuant to Section 50.c. of the 2016 Revised IRR of RA
9184;

WHEREFORE, premises considered, the __________ resolves to recommend to


the Head of the Procuring Entity (HoPE), the approval of procurement by Direct
Contracting for the Internet Service Provider of City/Municipality, Province in the
