AFFIDAVIT OF LOSS
I, REMEDIOS C. AQUINO, Filipino, of legal age, and a resident of
Northville, Calulut, City of San Fernando, Pampanga, after being duly
sworn in accordance with law, depose and state that:

1. I applied for a new Solo Parent ID after my old Solo Parent ID


with No. 2017-204 expired;

2. Since that time until the present, I have not received my new Solo
Parent ID;

3. Despite diligent search and efforts to locate the said ID with the
appropriate government office, I could not find the same, such
that I believe that it is now lost beyond recovery;

4. As such, I am executing this Affidavit of Loss to attest to the


truth of the foregoing and for any other legal purpose it may
serve.
