CLINIC VISIT

Clinic Visit is one of the community health nursing procedures that is done whenever
the patient visits the health center or clinic to avail the services there for consultation of one’s
health.
Registration or Admission: Greet the client upon entry and established rapport. The nurse must
then prepare the family records of new patient or retrieve records of old clients. He/she must
then elicit record the clients’ chief complaint and clinical history and perform physical
examination on the client and record it accordingly. Its purpose are number one, to avail the
services offered by the health center or clinic. Number two, for consultation and matters that ail
the client physically. Number three, to have a close interaction between health care providers
and the patient. All in all, clinic visit may enhance health education and promotion on health
care of the family in general.
Waiting Time: In waiting time, the nurse must give priority numbers to the client. He/she must
implement the first come first serve policy except for emergency and urgent cases. Emergency
or urgent cases should always be the top priority.
Triaging: This is the part where nurse manages program-based cases. Refer to all non-program
based cases to the physician. For all other cases which has no potential danger, treatment or
management is initiated by the nurse and she decides to do her own nursing diagnosis and
refer to the physician for medical management. In emergency cases, the nurse must first
provide first aid treatment and refer when necessary to the next level of care.
Clinical Evaluation: This is the part the nurse validates the clinical history and physical
examination of the patient. Identify the patient’s problem, formulate or write the nursing
diagnosis and validate, give and perform the nursing interventions and evaluate intervention if
it has enabled the patient to achieve the desired outcome.
Laboratory and Other Diagnostic Examination: The nurse must identify designated referral
laboratory when needed.
Referral System: The community nurse must refer the patient if he needs further management
to following two-way referral systems and accompany the patient when an emergency referral
is needed.
Precription or Dispensing: The responsibility of the community nurse in this part must give
proper instructions and drug intake.
Health education: Health education is the last part of the clinic visit. In this part, the nurse must
conduct one-on-one counseling with the patient, reinforce health education and counseling
messages and give appointments for next visit. Health education is done so that the patient can
still practice health promotion and disease prevention even at home or in situations where the
nurse or other health care providers are not around.
