NCP Anemia

The nursing care plan addresses a patient presenting with fatigue and activity intolerance due to anemia. Over the short term, after 8 hours of nursing interventions, the patient will report an increased activity tolerance and decreased physiological signs of intolerance. In the long term, months of nursing interventions will prevent weakness and complications while allowing the patient to return to normal activity levels. Nursing interventions include assessing the patient, recommending rest, monitoring lab values, and encouraging independent activity as tolerated while conserving energy.
