Dear Sir or Madame,

I received a summons to Jury Duty for (date). I am requesting an


exemption from Jury Duty because I am breastfeeding my infant. Per
Illinois Public Act 094-0391 Section 10.3: Any mother nursing her child
shall, upon request, be excused from jury service.

I would be happy to serve on a jury in the future but am unable to do so


at this time. I appreciate your understanding of my situation, and await
