Offer Letter of Employment

Date: September 10th, 2018

Dear Ms. Wresti Setyaningsih

On behalf of PT Mindray Medical Indonesia (hereafter referred to as "Mindray"), I am pleased to extend you this offer
of employment with our company. The terms and conditions are outlined as below:

Position: Sales Supervisor, East Indonesia


Location: Mindray Indonesia / Surabaya
Reporting to: Deputy National Sales Manager, National Sales Manager, General Manager
Date of Commencement: October 1st, 2018
Contract Period: 2 Years/ full time
Notice Period: One (1) months

Compensation: Annual Base Salary (gross) Rp195.000.000 (or Rp15.000.000 by 13 months,


including THR)
Annual KPI Bonus (gross) up to Rp30.000.000 (or up to Rp7.500.000 every quarter)
Annual Sales Bonus (gross) Rp150.000.000

Communication (gross) Rp6.000.000 (or Rp500.000 by 12 months)

Benefit: Statutory Social Securities (BPJS Kesehatan and Ketenagakerjaan)


12 days Annual Leave (pro-rate)
Group Staff Insurance (healthcare for employee, spouse, up to 3 children and personal
accident)

Cost and expenses not listed in this letter including but not limited to individual income tax, statutory insurance and pension
premium, medical expenses etc. shall be entirely borne by the employee himself.

You will devote your entire time and efforts to the Company’s business efforts and affairs and shall not engage in any other
business activity for remuneration or compensation without the Company’s prior written consent.

This offer is subject to our final endorsement upon reception of below documents. Please submit copies of the
following listed documents to us at your earliest convenience:

 Copy of ID Card (KTP), Family Card (KK) and Tax Number Card (NPWP)
 Copy of Social Security Cars (BPJS Kesehatan and BPJS Ketenagakerjaan)
 Copy of Passport
 Copy of academic and training certificates
 Passport-size digital photo
 Bank Account Number (BCA)
 Resignation Letter with company chop and signature endorsed by your most recent employer

If you agree to the terms and conditions set forth in this offer, please indicate your acceptance by signing on the space
provided below and return a copy to us before the expiry date. Any other matters not covered in this offer letter will be
subject to Mindray’s official employment agreement.

This offer letter is valid within five (5) working days from the date of issuance, and will elapse automatically if no
written confirmation received within the stipulated period.
