I, the undersigned EDWIN NUEVOS PAGAYONA is sending you this letter as


notification that I am allowing /authorizing the bearer of this letter, EDWARD NUEVOS
PAGAYONA, my brother to claim my PRC ID in your office.

I won’t be able to claim it personally because I am currently in Singapore and also due to
pandemic cause by covid-19, and I have asked him to bring this letter together with my
identification card as valid proof. I am also giving him the right to sign any papers that has
connection with claiming of my PRC ID.
