To Whom It May Concern:

The following inquiries were not authorized by me. Please delete them. I have never given my permission for
any company to inquire about my credit status. Any inquiries made are unauthorized. The presence of these
inquiries on my credit report constitutes inaccurate information, which under the Fair Credit Reporting Act
must be removed.

Once your investigation is complete please send me an updated copy of my credit report. I would appreciate
the name, address, and phone number of the companies who inquired about my credit so that I may follow
