making your nursing care plan. Use the separate sheet provided. Output should be handwritten. Write legibly.

Situation A:
Liza is a primigravidan. She just had her first baby delivered via normal spontaneous vaginal delivery with midline incision
episiotomy. During your nursing rounds, she complains of having perineal pains and is afraid to defecate because of her incision. What
will be your nursing diagnoses with this situation? How will you address her needs?

Situation B.
Marie, a G1P1, is on her 3rd day postpartum. She complains of severe pain in her breasts. You checked it and observed
breast engorgement. During your assessment, Marie verbalized, “I am scared. I’m afraid to overfeed my baby.” With this situation, how
will you handle it?

Situation C.

Gwen, a G2P2, just delivered her baby. She complains of pain in her suprabic area. During assessment, you have observed a
globular mass above the symphysis pubis. It has been 4 hours since she delivered her baby. To prevent further complications, what will
be your nursing interventions?
