Trinidad and Tobago 2021 Vaccination Initiative


Background
A pandemic is an epidemic of an infectious disease that has spread across a large region, for
instance multiple continents or worldwide, affecting a substantial number of people. A widespread
endemic disease with a stable number of infected people is not a pandemic.
The need to control the spread of epidemics from one country to another to protect
people's health and countries' economies led to the creation in December 1902 of what is today
known as the Pan American Health Organization (PAHO).
The Government of Trinidad and Tobago has initiated a vaccination drive to assist in
reducing the spread and mitigate the impact of the COVID-19 virus on the population. The
government uses only WHO approved vaccines – Pfizer, AstraZeneca, Sinopharm, and Johnson and
Johnson.
These vaccines will be distributed to the various Regional Health Authorities (RHAs) - the
North West Regional Health Authority (NWRHA), the North Central Regional Health Authority
(NCRHA), the South-West Regional Health Authority (SWRHA), the Eastern Regional Health
Authority (ERHA), and the Tobago Regional Health Authority (TRHA) for distribution to the
population. Each RHA will be responsible for the distribution at various sites under their purview.
The cost of the vaccine to the population will be free, but the RHAs have to account for the vaccine
cost. The table below indicates the cost to the RHA for each type of vaccine :

Until vaccination is widespread among the population, basic public health measures remain
the basis of the pandemic response.
For public health authorities, this means continuing to conduct screening, contact tracing,
isolation, assisted quarantine, and quality care, and for people, it means continuing to practice
physical distancing, hand hygiene, the use of masks, adequate ventilation of indoor environments,
and avoidance of crowded spaces.

Description of the Project


The Ministry of Health (MoH) is responsible for managing the distribution of the vaccines to
the RHAs and also has been tasked with recording the vaccination status of the population.
The MoH has hired you to simulate the data collection process of the Trinidad and Tobago
Vaccination Initiative in each of its RHAs.
You are required to utilise suitable word processing, web design, spreadsheet, database
management as well as a programming application to design and implement computer-based
solutions to the tasks involved in the simulation of the pandemic vaccination data collection process.
WORD PROCESSING – 20 marks
1. (a) Using appropriate document formatting features, you are required to write a letter (used
as a template) to each of the five (5) RHAs indicating the vaccine name, and the number of
doses assigned to the RHA.

Perform a Mail Merge with a relevant data source using your word processing
program and the letter created in 1. (a).

The letter must contain relevant Merge fields for Name of RHA, Address of RHA,
Vaccine name and the Assigned Doses.
A relevant graphic must be included in the header of the letter indicating MoH. The number
of each vaccine allocated to the RHA should be included in a table.

Set the margins of the letter to 2”. Use justified alignment and double line spacing for your
letter. Italicise and bold the RHA address. Ensure that your letter fits ONLY on one page.

(b) A Pre-registration vaccination form has been requested from the Chief Medical Officer of the
MoH.

Design a Fillable Form to record the pre-vaccination details about the citizen. Utilize fillable
form controls for the appropriate, suitable and relevant fields on the form. The form should be named
“COVID-19 Vaccination Pre-Registration Form”.

The form should have at least the following fields:

Date (Date picker Content Control); RHA Name (dropdown list option); First Name (text box
content control); Last Name; Identification Type (choice of either ID DP BP PP); Identification No.;
Gender (checkbox control); Date of Birth (dd/mm/yyyy); Age; Address; Contact No./Email; Next of
Kin; Next of Kin Contact.

Please ensure that your form is completely designed and has all aforementioned fields.
WEB DESIGN – 10 marks

Use either Microsoft Word OR a free online web service (e.g. Google sites, Weebly.com, wix.com) to
design a website.
Details of your site are as follows:
The MoH has asked that you design an informational and interactive Covid-19 website using:

1. A logo (of your design/choice) and background image (creative commons) in the
health sector.

2. Home page: describes a brief history of the pandemic in your country and the public
health initiatives implemented.

3. Add a New page, “Vaccination Information at the RHAs” that provides information
about the locations of vaccine distribution sites within each RHA. You can add
image(s), narrative, videos and insert a google map showing general locations of
distribution sites.

4. Use appropriately labelled hyperlinks to the email addresses of each of the RHAs.

5. Use a hyperlink that connects to the World Health Organisation (WHO) website,
where persons can get additional information about each of the vaccines being
distributed.
SPREADSHEET ASSIGNMENT -20 marks

1. You are required to design a spreadsheet that stores data on the citizens recording data about
their vaccination status. The sheet should be similar to the image below:

ID# is a 4 digit number
Age – to be calculated
RHA – drop down list with the RHA abbreviated names – NWRHA, NCRHA, SWRHA, ERHA, TRHA

Co-morbodities – drop down list with the following – Hypertension, Diabetes, Heart Disease, Asthma,
Obesity, None
Vaccine Type – drop down list – AstraZeneca, Johnson & Johnson, Pfizer, Sinoopharm
1st Dose, 2nd Dose – drop down list – Yes, No.

Create data entries for at least 20 citizens.

Include a heading above the table “Pandemic Vaccination Data”. Center this heading across
the table.

a) Using an appropriate pre-defined function, determine the Age of each of the citizens
and insert into column for Age. (HINT – You should use a function to add today’s date
in another part of the spreadsheet for you to access.)
b) Using the excel Logical IF function, indicate the person’s Vaccination Status. Fully
Vaccinated if they have received both doses or Not Fully Vaccinated if they only
received the 1st Dose.
c) Create a summary table showing the number of each vaccine distributed at each RHA.

Name your worksheet Vaccination01.

Copy the Vaccination01 worksheet and paste it into another blank worksheet. Name this new
worksheet Vaccination02.

2. a) Using Arithmetic functions show:


1) The average age of persons in the table.
2) The Oldest person to have received the vaccine.
3) The total cost of vaccines distributed.
b) Select the DOB and Age fields and sort the worksheet so that the Age field is in
Descending order ONLY.

c) Perform a simple/auto filter on the column RHA to show the vaccines distributed at
the SWRHA. Show the result of this filter on a separate worksheet.

d) Perform an advanced filter to find those citizens who received their 2nd Dose and are
older than or equal to 60 years of age.

e) In another worksheet, create a one-dimensional pivot table to show the results of


summarizing the data from the columns RHA, Vaccine Type, Cost of Vaccine. This will
show a summary of the total cost of each vaccine type distributed at each RHA.

Name your worksheet Vaccination Pivot Table.

3. In another worksheet design a chart to represent the amount of each vaccine


distributed across all RHAs. (Hint: Use the summary table created in Q1 above)

Give your chart the title “Vaccination Type by RHA”.

Name this worksheet Vaccination Chart.


DATABASE ASSIGNMENT – 20 marks
You are to use a suitable application to manage data on all citizens tested in the general
population. Three tables/relations must be used to store the data.
Create a table named Citizen_Bio_Data containing relevant personal data on each citizen as
follows: Citizen ID (Unique), First Name, Last Name, Date of Birth, Age, Gender, Blood Type,
Comorbidities or Chronic ailments, RHA ID
Create a second table named Vaccine_Dosage_Data with the following fields:
Vaccine ID (unique), Vaccine Name, Citizen ID, 1st Dose, 2nd Dose

Create a third table to show the number of each vaccine type that were assigned to each RHA
named RHA_Info with the following fields:
RHA ID, RHA Name, Address, Pfizer Doses, JJ Doses, AZ Doses, Sinopharm Doses

Populate your tables with multiple entries. Use at least 20 citizen records. Use appropriate
field names with relevant data types for each table. Ensure that each table has a primary
key. Create relationships between your three tables.
You should test your Database by generating queries as follows:
1. a) Create a simple query using the Citizen_Bio_Data table of all citizens who are 60
years and over and have comorbidities/chronic ailments. Name this query High Risk.

b) Create a query that generates the names of all citizens who have received their 1st
Dose of a specific vaccine only. Name this query Awaiting Second Dose.

c) The Government has secured a second set of vaccine doses, and has increased the
allocation to each of the RHAs for each vaccine by 1.5 the previous allocation. Create
a calculated query to show the increased allocation to the RHAs. Name this query
Increased Allocation.

d) Design a Form using the Citizens_Bio_Data (main form) table with a sub-form using
the Vaccine_Data table for all citizens. Name your Form Vaccine Citizen Tracker.

e) Prepare a report, which lists the names of all citizens who have received both
vaccine doses.
The report must show the fields for Citizen Name, Age, Gender, RHA and
Vaccine Name.
The report must be sorted (ascending order) on Last Name and grouped on
RHA and then by Vaccine Name. The summary value of total number of doses
distributed must be shown at the end of the report. Name the report “Vaccine
Distribution Status”.
PROBLEM-SOLVING AND PROGRAMMING – 15 marks

1. Develop an algorithm using flowchart or pseudocode to determine the overall results


of vaccinations performed on all citizens.
The algorithm should accept as input the Citizen ID, Citizen Name, Vaccine
Name, Dose 1 and Dose 2. (NB. Dose 1 and Dose 2 are variables use to hold yes/no
responses). A Citizen ID of 0000 indicates the end of the data.
Those citizens that received the vaccine for the first time are now partially
vaccinated and an appropriate output message should be displayed stating that they
are to return for the second dose at a future date.
Those citizens receiving the 2nd dose, are now fully vaccinated and an
appropriate output message should be displayed indicating that they are fully
vaccinated and can now return to the wider population safely.
In the end, the algorithm should display a summary of the number of citizens
that received only the first dose of a specified vaccine. Number citizens that received
both doses of a specified vaccine.

2. Design and execute a trace table that accepts data for all citizens undergoing nationwide
Pandemic vaccination drive. (Your table should show iterations for the data set provided by
your teacher and all variables used).
3. Using the PASCAL programming language, write program code to implement the
algorithm above.
