AFFIDAVIT OF CLOSURE OF BUSINESS

I, CARMELO SOLVER GARDONA, of legal age, single, Filipino citizen


and a resident of 18 C Hernani Street Laloma, Barangay Salvacion, Quezon
City, after being duly sworn to in accordance with law do hereby deposes
and says:

1. That I am the authorized person to sign, process and secure for the
business closure of Carltam Pawnshop located at 721 Barrera Street,
Barangay Poblacion, Baliwag, Bulacan, in behalf of its legitimate
owner, Ramon C. Tambunting, a copy of Secretary’s Certificate is
hereto attached and marked as Annex “A”;

2. That above-mentioned business has not been operational due to its


closure last 3 December 2021;

3. That above-mentioned business has been closed because of low profit;

4. That I am executing this affidavit in order to attest to the truthfulness


of the foregoing and for whatever legal intents and purposes it may
serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 22nd day


of December, 2021, at Baliwag, Bulacan.
