Causes of Flexible Endoscope Damage and Suggested Repair

This document discusses common problems with flexible endoscopes and their potential causes and suggested repairs. It addresses issues such as poor water/air flow, suction, irrigation, biopsy channel damage, angulation problems, fluid invasion, lens staining, and broken images. The potential reasons for each problem include damage from instruments, wear and tear, improper cleaning, chemical buildup, and more. The suggested repairs involve replacing components like nozzles, valves, channels, bending sections, sheaths, buttons, and light bundles.

Flexible

endoscope
Problems &
suggested repair

Problem - Poor Air/ Water flow

Water is not shooting over objective lens

properly. Air or water nozzle is faulty or

missing

POSSIBLE REASONS
Scope is not cleaned immediately following

procedure.
Nozzle is damaged, missing or misaligned.
Glutaraldehyde build-up from chemical

disinfectants can break away from the channel

and clog the air/water nozzle.

SUGGESTED REPAIR
Air/water nozzle or channels replacement or

atleast unblocking / unclogging is needed.

PROBLEM POSSIBLE REASONS
A groove is worn in suction port,
Poor Suction suction is decreased or Blockage/
resistance is felt in the suction
channel.

Blockage and/or wear and tear

SUGGESTED REPAIR
Suction Channel Replacement
Problem - No irrigation / No

insufflation

POSSIBLE REASONS
Valve may be stuck or occluded

SUGGESTED REPAIR
Lubrivate Valve (O-rings)
Valve replacement
POSSIBLE REASONS

PROBLEM Damage to Biopsy Channel is usually

caused because of:

Instruments do not pass or Kinked, damaged or open Flexible

resistance is felt in the channel. Endoscope biopsy forceps causing tears

Buckles or tears are seen in the in the channel material.

Buckling of the insertion tube can also

channel material.
cause buckles in the channel.
Forcing instrumentation through the

channel can cause wear or tears in the

channel material. This frequently occurs

in the bending section when resistance

is met when the scope is angulated. Do

not pass anything through the bending

section with the tip is angulated further

than 110°.

SUGGESTED REPAIR
Biopsy Channel Replacement
Problem
Angulation is tight or not proper.

Bending section may bend irregularly

or to one side

POSSIBLE REASONS
Forced angulation may lead to stretch or break

in angulation wires.
Buckling of the insertion tube can also stretch

and break wires.

Play in the angulation control knobs usually

indicate an angulation adjustment is needed.

SUGGESTED REPAIR
Angulation adjustment / angulation wire

replacement Bending section mesh or articulating

section replacement
PROBLEM
Clicking and/or sluggishness in the POSSIBLE REASONS
angulation controls may be seen Bending sheath damage

SUGGESTED REPAIR
Angle drive rebuild
Problem
Fluid Invasion

POSSIBLE REASONS

Fluid invasion – If small leaks are found in bending

rubber, insertion tube, light guide tube, body etc.,

anywhere on endoscope, this may cause Fluid

invasion.
Tears or holes in the sheath material are caused

by:
Any sharp objects like instruments, fingernails or

bites
Normal wear and tear
Over inflation can cause stretching or looseness

of the bending rubber material.

Not following sterilization process properly.

SUGGESTED REPAIR

Immediate endoscope repair or re-sheathing

PROBLEM POSSIBLE REASONS

Stains, cracks, intereference in the Major fluid invasion , which has

images, debris on the lens does not resulted in video chip damage,
go with wiping. corrosion of internal metal parts, fluid
at the distal end

SUGGESTED REPAIR
Immediate endoscope / CCD
repair
Problem
Button not working or stuck causing

constant beeping

POSSIBLE REASONS

Fluid invasion causing damage to button

SUGGESTED REPAIR

Button Replacement
Problem
Break in images

POSSIBLE REASONS

Buckles / bites in the insertion or light guide tubes

Dropping scopes
Damage in distal end
Fluid invasion causing brittleness in fibreoptic

bundle

SUGGESTED REPAIR

Light guide Illumination bundle needs replacement.
