DepEd Privacy Notice: The provided email address will be recorded upon submission of this

form.  All information that you will provide will only be used for this particular program. The
Department of Education recognizes its responsibilities under the Republic Act No. 10173 (Act),
also known as the Data Privacy Act of 2012, concerning the data they collect, record, organize,
update, use, consolidate or destruct from their personnel. The personal data obtained from this
form is entered and stored within DepEd's authorized information and communications system
and will only be accessed by the authorized personnel. DepEd has instituted appropriate
organizational, technical and physical security measures to ensure the protection of personal
data. Furthermore, the information collected and stored in the portal shall only be used for this
activity. DepEd shall not disclose any personal information without their consent and shall retain
this information over ten years to effectively implement and manage its activities.
