1. Section 8 of Republic Act 6713 (Code of Conduct and Ethical Standards for
Public Officials and Employees) provides that all goverrment officials and
employees shall file their Sworn Statement of Assets, Liabilities and Net Worth
(SALN).
2, In this regard, all employees are required to submit to the Division Office their
Sworn Statement of Assets, Liabilities and Net Worth (SALN) as of December
31, 2022 on or before February 1, 2023.
3. The currently prescribed SALN fomi is the one revised as of January 2015 per
CSC Resolution No. 1500088 promulgated on January 23, 2015.
4. It is requested that the submission thereof be by School for the Elementary
and Secondary Schools in two (2) copies (separate folders). While the
employees of the Division Office are required to submit individually and also
in two (2) copies,
5. Schools Heads are advised to evaluate as to the completeness of the
accomplished SALN forms submitted by the employees.
6. Herewith is the guide in Accomplishing the SALN F`orm, Template of the
Certification of Review and Compliance and the Template of the Certification
of the electronic transmittal of the submission of the SALN.

1. December 31, 2022 or End of December 2022 should be indicated in the first
page, upper portion of the form;
2. If the Declarant and spouse are both working in the government, the tiox "Joint
Filing" or ¢Separate F`iling" must be ticked with a check mark. A Declarant who
is single or whose spouse is worldng in the private sector shall tick with a check
mark the "Not Applicable" box;
3. Declarants who are married must indicate the name of their spouse in the space
provided in the form;
4. Information that are not applicable should be supplied with "N/A". "Nothing
follows" is not acceptable;
5. The exact location must be indicated in the Real Hoperties column, i.e. house
number and street, pursuant to the revised SALN fomi as of January 2015;
6. Assessed and Current Fair Market Value, whether owned or mortgaged, should
be indicated in the Real Property Assets and it should be based on what is stated
in the Tax Declaration of Real Property;
7. Mortgaged properties are already under the name of the Declarant. Hence, the
mortgaged properties shall be declared either under real or personal properties.
The acquisition cost to be declared shall be the actual purchase price. However,
the declarant should declare the outstanding balance of the mortgage loan as of
December 3 1 of the preceding year under Liabilities
8, Declarant's children who are 18 years old and above should not be included in
the list even if they are still living and receiving rmancial support from the
Declarant;
9. Name of spouse worldng in the government should not be included in the
"Relatives in the Government Service" since it is already reflected in the "Spouse"
section;
10.After filling out the form, the spouses may reproduce the SALN Fomi as the
number of copies is required, but their signatures should be original in the SALN
Fomi to be submitted to the Ofrice;
11.Both spouses should sign, whether in the government or private sector. If
signature cannot be acquired from the spouse, an explanation should be attached
or indicated in the space provided for the signature of the spouse.
