4. Type the Filename of the form, same with the Title. Write the Form Description
(Instruction/s)
5. On the Question No.1, type the question, set as Multiple Choice, Type the responses in
an alphabetical order, click required button for them not to skip to the next question
without answering the previous question. You can add Picture, by clicking Add Image
button, Drag or upload picture.
6. To customize theme, Click the customize theme button, Choose Image from the default
themes or you can upload image for your own image, then click Insert. Select theme
and background color.
7. Click Settings, uncheck restrictions so that everyone with the link can view and answer
the google forms quiz, set presentations to edit confirmation message or you can use
the default confirmation message, and activate the (make this a quiz) bar, then click
Save.
8. For sending the file for your learners or responders, Click Send, Click the clip button,
copy the link, or Shorten URL. You can shorten also URL through Tinyurl.com. Copy the
link, paste it to Enter a Long URL button, customize it to the name of the activity, then
click Make TinyURL. Copy the URL to share. To send the form, click Send button.

Note: You can also share the default link. The dialog box of Send Form will appear, click
the Link Icon, the URL address will display. Activate the Shorten URL, then click Copy
9. To easily access and view the responses, Click Responses, Select Create a new
spreadsheet, then Click Create
10. You can share the link so that you can view easily their responses and for your
responders to know if their responses submitted has been recorder. You can make
TinyURL for this.
