For Cash Investment
The purpose of Cash Investment Application form submitted:

❖ For cash investment only.

❖ For cash investment into Public e-Series of Funds eg: PeAITF, investor must be a PMO
subscriber.

❖ Submit to PUBLIC BANK together with cheque. 1 cash form 1 cheque.

❖ If have any amendments, cannot use corrective liquid. The amendment must cross &
countersigned by investor.

❖ Use black ink pen only.

❖ UTC are not allowed to collect cash. Only can accept either cheque / banker cheque.

❖ The front image of cheque should be written: Public Mutual Berhad (NRIC investor) &
back image of cheque should be written Name of Investor, IC number of investor &
contact number. If NRIC investor not in the cheque – bank will reject.
