1- The GWM Company did not trained three apprentices to use of the crane. The
company must ensure that all workers instructed and under their control.
2- The GWM does not believe that they should be responsible for health and
safety, and this is a big mistake. The company should provide safe workplace,
safe equipment and safe system of work and good training and supervision to
prevent any accidents.
3- Ignoring the email from supervisor about training the apprentices
Crane operation training for the apprentices, but they ignored the email. They
should provide effective communication and consultation with worker.
4- The GWM is not interested in people moaning about insignificant issues, which
makes workers believe their views not heard. They should hear their worker to
prevent any risk to happen.
5- Lack specific details about risk assessment, and completed three years ago. This
is lack of proper information and unsafe system of work.
6- GWM said there is no money for the health and safety in the workshop, it is not
true, they should prioritize health and safety.
7- The apprentice was using his mobile at the workplace; the company should give

8- When steel beam bent down, the apprentice raising their arms to avoid it. The
beam struck their forearm, which caused them to fall onto some scrap metal.
The company should provide performance monitoring and good supervision.
9- The injured apprentice had broken their arm in several places and required an
operation. Related to article 16 of c155 The GWM company should provide PPE
and protective equipment to make sure all worker safe at workplace.
10- The GWM Company carried out investigation in a few weeks. They should
investigate promptly to determine the root causes of accident know the
mistakes and prevent of occurring again.

- Their position or involvement in the accident, the apprentice was using their mobile
phone, and the apprentice who operate overhead crane
- Their actions immediately after the accident, the apprentice forced to move outside of
the dedicated walkway and into the path of the steel beam, and raising their arm to
avoid steel beam bunted down. First-aider says that the apprentice broke their arm, and
treated a deep cut on the apprentice’s leg. The first-aider reported this to one of the
workshop supervisors, who then called for an ambulance.
