AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, _______ and ________ of legal ages, Filipino citizens and with
residence address at ______, Norzagaray, Bulacan, after having been jointly
duly sworn to in accordance with law, do hereby depose and state that:

1. We are neighbors and friends of the common-law-spouses EL C. A and


the late AR JR.

2. We know that during their cohabitation they were blessed with two (2)
children namely:.

3. We also know the fact that before the spouses cohabited to each other
have four (4) children to her previous spouse.

4. We discovered that in the Certificate of Live Birth of JUNEL and R,


the Birth Order was mistakenly entered as 1st and 2nd which was
supposedly to be 5th and 6th, respectively.

5. That the said error was an honest mistake of fact considering that the
informant was the father and he believed that the Birth Oder was
reffering to his child which was his 1st and 2nd child respectively,
without adding the previous children of his wife to her former husband.

6. That we know the foregoing facts considering that we were neighbors,


friends and acquantances of the family of the said spouses.

7. We are executing this affidavit in order to attest to the truth of the


foregoing facts and circumstances to all legal intents and purposes.

IN WITNESS WHEREOF, we have hereunto set our hands this 3rd day
of March 2020 in the Norzagaray, Bulacan.
