The questions posed within this questionnaire are extracted from the subject matter contained
within this study guide. In many instances the answer to the question is contained within the
study guide with the exception where the subject matter relates to a process and/or procedure
specific to a workplace. In such instance your facilitator will provide you with additional
information (which may include additional notes) enabling you to provide answers.

The purpose of this questionnaire is to determine if you have developed the essential
knowledge and skills necessary to lead a workplace team, where the use of critical knowledge
and underpinning skill are essential in the furtherance of your skill development.
You may use your study guide and/or other reference material provided by your Facilitator in
finding answers to questions.
Question 1:
Provide a general summary of your role as a carer.
Explain how an individualised care plan will assist you in providing the
support needs of an individual without overstepping your role
boundaries.
It is my responsibility as a caretaker to support those who require help with daily living
activities. In order to analyse their needs and create a personalised care plan that satisfies
their unique requirements, I will work closely with the person. This care plan will explain the
assistance offered, allowing me to render the required assistance and support without going
beyond the bounds of my duty. I can modify the care plan to meet the individual`s unique
demands by taking into account their wants, preferences, and lifestyle. This will make it
possible for me to offer the finest service in a respectful and safe manner.

Who is involved in the writing of an individualised care plan?
What input is sought from the individual in care?
Why is it important that you confirm the content of the care plan with
the individual, family or health professionals/advisers?

Why is it essential to ensure that your clients are aware of their rights?

balance the duty of care requirements with the dignity of risk
requirements?

support/care.
Provide five (5) examples of where activities with a client requires the
preservation of dignity and privacy and describe how you can facilitate
such?

Provide three (3) examples of engagement with a client where you are
not able to provide the support requested? For each example identify
who you will engage to provide the support requested?

As a support person to a client, how can you monitor your activities to
ensure that the client is receiving the best possible support?

Continual support of a client over an extended period of time may
result in changes to the support plan or more intense support provision
for a client. Whose responsibility is it to inform appropriate persons of
changing circumstance that might need a care plan review? What is
your role in such a review?
