Reflective log 02

Description:

In a clinical placement, I got an opportunity to work in an isolation room of pulmonary war . I was
assigned a task to do medications for the patient. I give medications to the patient using hospital and ward
rules. Our mentor supervised me during the process of administering medication. There were also four
pants of blood needed to administer to the patient. Now the mentor came forward and started blood
transfusion to the patient without wearing to the gloves and washing hands before. I thought for a moment
about what to do or say. After a while I dared to say something, because it was our first day in this ward.

Feelings:

I was alarmed by this, as I had expected the mentor to wash his hands and put on gloves before doing any
procedure. However, I felt intimidated because I felt that my mentor was more experienced than me as a
4th year nursing student; and I didn't want to embarrass him. Also, I didn't want to make old patient
concerned by confronting the doctor in front of him.
Later, I spoke to the head nurse about the incident. He encouraged me that we will together discuss this
issue with your mentor. When he entered the office the head nurse told him about the mistake. He
accepted the mistake and assured him to not repeat it again.

Evaluation

The incident was extremely challenging for me. We need to be equipped with PPE specially while
working in isolation ward. I was very glad to see that my suggestion is respected and taken into account
by head nurse. The mentor accepted the mistake without any arguments, which gives us a beautiful lesson
to learn from mistake and accept your mistake before you harm yourself and patient.

Analysis

Hand hygiene is the most important part of practice for health care workers and is the single most
effective way to stop the spread of infections; failure to properly perform hand hygiene is the leading
cause of HAIs and the spread of multi-drug-resistant organisms (MDROs) (BC Centre for Disease
Control, 2014; WHO, 2009a).  Gloves are critical to prevent the transmission of organisms when hand
hygiene alone is not enough in an outbreak such as Clostridium difficile or the norovirus, or when a
patient has a suspected or known pathogen. Studies have shown that gloves reduce transmission of
microbes from the hands of health care workers (PIDAC, 2012).
The Nursing and Midwifery Council Code of Professional Conduct (2004, section 8) states that as a
nurse 'you must act to identify and minimize the risk to patients and clients'. As the student nurse caring
for old under my mentor's supervision, this also applies to my own practice as a student nurse. 

Conclusion

From this incident I came to know that we should need to report any malpractice that harm the healthcare
provider and patient. When we address such ignorance and apply the standard protocol during procedure
ours and patient safety will be protected.

I realize that I need to be supportive to colleagues, understanding the pressures that they may be under,
but ensuring that their practice does not put clients at risk. 

Action plan

We will try our best to learn the procedures at well so that to avoid mistake as much possible. I will gain
all these skills and then apply it under the supervision of my seniors. In future, I will aim to develop my
assertive skills when working with colleagues, in order to ensure that the well-being of clients is
maintained.
