AFFIDAVIT

I, Vince Quicho Matutina, of legal age, single , Filipino and


a resident of #22 Manansala Street Barangay Dadiangas North General Santos City
having duly sworn in accordance with law,respectfully aver that:

1.That in my authenticated Certificate of Live Birth issued by the National


Statistic Office and Register of Births Record in the Office of the Civil
Registrar of the City of General Santos ,Philippines,it was stated that I was
Born on 17 January 1996 at Saint Elizabeth Hospital, General Santos City
Philippines;

2.That I have used 17 January 1996 as my birth date in all pertinent school,
organization and voter’s registration records;

3. That when I filled up my application form for the October 2, 2022 Licensure
Examination for Teachers (LET) before the Professional Regulation
Commission, I had correctly written my day and month of birth January 17;

4.That when I passed the licensure examination for teachers , I search my rating
using Licensure Examination and Registration Information System I discovered
that I had inadvertently and erroneously written my date of birth at January 17
2010 instead of 17 January 1996;

5. That filing the Petition for Correction of Date of Birth from January 17, 2010 to
January 17 1996 was made lapsed of time since to I have gone to PRC davao
twice because I forgot my Birth Certificate in my first application of examination
The second time I brought my birth certificate they just corrected my Middle
name. The error in my birth January 17 2010 remain uncorrected despite I have
shown them my birth certificate three times also My examination permit did not
reflect my birthday only my name so I did not notice it;

6.That I am executing this affidavit to attest to the truth of the foregoing facts
and explain the discrepancy in my Certificate of Live Birth , Voter’s
Certificate and professional driver license as regards to my date of birth;

7.Further , I am Declaring that from this date hereon I would be using my true
and correct date of birth of 17 January 1996 in all my papers and other
Documents.
