Re:Mrs Sally Fletcher, Aged: 25 years
Your patient,Mrs Fletcher, who has underwent laparoscopic surgery on 25/3/2018 resulting from
endometriosis. She will be discharged today and will requires to follow up with you for further
care.
Mrs Fletcher presented to hospital with frequent acute menstrual pain which localised at the
lower left quadrant of abdomen despite taking OTC(naproxen).Occasionally, she reported of
constipation that associated with pain in left lower quadrant of abdomen.Transvaginal ultrasound
was ordered and revealed 6 cm cyst, which is consistent with endometriosis, for which
laparoscopic surgery was performed on 25/3/2018.Post operatively,no complications were noted
and she has recovered well.
During her hospital stay, post op mobility is permitted on 26/3/2018.However,driving is prohibited
when she is on analgesic, but driving can be resumed after her nal dose of analgesics. Her
nursing management focused on ambulating as tolerated and also to encouraged her to take
orally.
It would be greatly appreciated if you could provide Mrs Fletcher for further care.
If you have any questions, please don’t hesitate to contact me.
Your sincerely,
Doctor.
