APPLICATION FOR SEEDING AADHAAR NUMBER, RECEIVING DIRECT BENEFIT TRANSFER (DBT) AND FOR ENABLING

Sub: Seeding of Aadhaar for receiving Direct Benefit Transfer (DBT) into New Bank account / Existing Bank account with HDFC
Bank, _______________________________ Branch
(1) I am desirous of receiving my entitled benefits of subsidies for welfare schemes funded from the Consolidated Fund of India or the
Consolidated Fund of State.
(2) I wish to seed my Aadhaar number for receiving my entitled benefits of subsidies (as mentioned in point no. 1 above) in the below
selected HDFC Bank account (TICK ANY ONE):
 New Bank account - I wish to open a New Bank Account with HDFC Bank’s _______________________________ Branch.
 Existing Bank account - I am maintaining a Savings Bank Account with HDFC Bank.

(3) I have been given other alternative means by HDFC Bank for KYC purposes including physical KYC by submitting officially valid
documents other than Aadhaar. However, since I am desirous of receiving my entitled benefits of subsidies i.e. Direct Benefit Transfer
(DBT) in my Account as mentioned in point no. (1) & (2) above, I have voluntarily chosen Aadhaar based KYC or e-KYC or Offline
Verification, and submit to the Bank my Aadhaar number, Virtual ID, e-Aadhaar, XML, Masked Aadhaar, Aadhaar details, demographic
information, identity information, Aadhaar registered mobile number, face authentication details and/or biometric information
(collectively, “Information”).
(4) I authorise and give my consent to the Bank (and its service providers), for carrying out my identification, offline verification or e-KYC
or Yes/No authentication, demographic or other authentication / verification / identification in accordance with applicable Law.
(5) For the aforesaid purposes, I submit voluntarily and at my own discretion, my Aadhaar number as issued by Govt. of India, to HDFC
Bank and voluntarily give my consent to seed my Aadhaar number in my Account (as mentioned in point no. 2 above) and to use my
Aadhaar details for authentication with UIDAI.

(6) Options for Seeding Aadhaar Number and Receiving Direct Benefit Transfer (DBT) in HDFC Bank account (TICK ANY ONE):
 My Aadhaar number is NOT Seeded with any Bank: I wish to seed my Aadhaar number in my Bank Account and with NPCI Mapper
to enable me to receive DBT including LPG Subsidy from Govt. of India (GOI) in my HDFC Bank Account. I understand that if more
than one benefit transfer is due to me, I will receive all the benefit transfers in the same Account.
 My Aadhaar number is already Seeded with another Bank: I already have an account with _____________________________
_______________ (Name of Bank) having IIN Number **__________________ and seeded with NPCI Mapper for receiving DBT
from GOI. I request you to change my DBT Benefit Account (NPCI mapping) to my Bank Account with your Bank’s
_______________________________ Branch.
(7) I have been explained in local language about the consent, purpose of collecting Aadhaar and the nature of information that may be
shared upon authentication by UIDAI (Aadhaar details). I have been given to understand that my information submitted to the Bank
herewith shall not be used for any purpose other than mentioned above, or as per requirements of law.
(8) HDFC Bank has informed that my biometrics will not be stored / shared and; Aadhaar details will be submitted to Central Identities
Data Repository (CIDR) only for the purpose of authentication for the aforementioned purpose. HDFC Bank has informed me that this
consent and my Aadhaar number will be stored along with my account details within the bank in accordance with applicable Law.
(9) I authorise the Bank to enable my account (as mentioned in point no. 2 above) for Aadhaar enabled Payment System (AePS)
transaction services.
(10) I have downloaded the e-Aadhaar document myself using the OTP received on my Aadhaar registered mobile number.
(11) I hereby authorise & consent to HDFC Bank to collect, disclose, share, store, preserve and use the Information and authentication data
and records, as may be required under applicable laws or for the purposes above or as per the internal requirements of HDFC Bank.
(12) I hereby declare that all information voluntarily furnished by me is true, correct and complete. I will not hold HDFC Bank or any of its
officials responsible in the event Aadhaar is not found to be in order or in case of any incorrect information provided by me.

* NPCI (National Payments Corporation of India) Mapping: It is a process by which the Bank’s name / IIN and customer’s Aadhaar number is updated
in NPCI Mapper for enabling the customer’s Bank account to receive DBT / subsidies.
** IIN (Issuer Identification Number) is a number used to identify Banks and will be updated by the Bank receiving the consent Application from the
customer, basis the previous Bank details i.e. “Name of the Bank”.
