I, ___________, Filipino, of legal age, married and resident of __________, Philippines,


after having been duly sworn to in accordance with law hereby depose and say:

1. That I am the owner/operator of a certain motor vehicle which is more particularly


described as follows, to wit:

2. That on May 25, 2014 as per Decision, I was granted a Certificate of Public
Convenience by the Land Transportation Franchising and Regulatory Board
(LTFRB-Region 7), Cebu City; thus, the above-described motor vehicle was issued
authorization to operate the same as VEHICLE FOR HIRE;

3. That subsequently, I filed a Petition for the Dropping of the registration of the motor
vehicle from FOR HIRE to PRIVATE. Consequently, LTFRB-Region VII granted
the Petition on March 9, 2020 as per Order for Dropping;

4. That the registration of the foregoing motor vehicle has been changed from
VEHICLE FOR HIRE to PRIVATE;

5. That I execute this affidavit freely and voluntarily to attest to the truth of the
foregoing fact stated herein and for whatever legal intents and purposes this may
serve;

IN WITNESS WHEREOF, I have hereunto set my hand this 7 th day of July, 2020 in the
