experience to prove my qualifications for overseas employment;

That the original documents hereto attached are genuine, authentic, and sufficient
evidence of the averments therein contained / That hereto attached Certified True Copies
of the documents are complete and faithful reproduction of the original documents;

That I hold myself liable for any case of perjury, falsehood, misrepresentation, or
any act of dishonesty should it be proven that the documents submitted are fraudulent
and/or tampered with;

That I am fully aware that any false representation is a ground for the denial of my
Visa Application, or cancellation of Visa once issued, ban on future overseas
employment and criminal prosecution;

That I am executing this Affidavit to attest to the truth of the foregoing and in
support of my Visa Application as a proof of my qualifications for overseas employment,
and for whatever legal purposes this may serve.
