He stated that Mrs. Mary Obageli Isidore Okpewho was his aunt and the wife of Mr. Isidore Okpewho, who passed away on September 3, 2016. As the next of kin to the late Mr. Isidore Okpewho, Mrs. Mary Obageli Isidore Okpewho has authorized Mr. Sojeh Azuka Tony to process any death benefits through this affidavit.

I, MR. SOJEH AZUKA TONY (M) Christian, Citizen of Federal Republic of
Nigeria, and residing at No 20 Iyabi Mathilda Njokamma Street, Asaba, Oshimili
South Local Government Area of Delta State do hereby make an oath and state as
follows:
1. That I am the deponent herein and lawful attorney to Mrs. Obiageli Isidure
Okpewho
2. That Late MRS. MARY OBAGELI ISIDORE OKPEWHO is my aunty.
3. That Late MR. ISIDORE OKPEWHO was my In-law
4. That MRS. MARY OBAGELI ISIDORE OKPEWHO was the wife of
LATE MR. ISIDORE OKPEWHO who died on the 3rd day of September,
2016, at Asaba, after a brief Illness.
5. That MRS. MARY OBAGELI ISIDORE OKPEWHO is the next of Kin
to Late MR. ISIDORE OKPEWHO
6. That I therefore make this Affidavit to enable me to process his death benefit
from the Authorities concerned.
7. That I make this Affidavit for record purposes, for the information of
probate Registrar, the general public and TO WHOM IT MAY
CONCERN to please take note.
8. And that I make this solemn Declaration in good faith, conscientiously
believing same to be true and correct by virtue of the Delta State Oath Law
of 2006.
