PETITION
The Petitioner, NESTOR CRISPIN MIGUEL B. LUMAPAS,
(hereinafter referred to as Petitioner for brevity), unto this Honorable
Court, most respectfully states that:
1. Petitioner is of legal age, Filipino, single, and a resident of Block 5,
St. Peter St., Hermag Village, Basak, Mandaue City;
2. Petitioner was born on 27 June 1990 in Mandaue City;
3. Petitioner completed and finished his Bachelor of Laws (LlB)
degree from the University of San Carlos (School of Law and
Governance) in March 2015. He became a member of the
Philippine Bar having passed the 2015 Bar Examinations and
admitted as such on 17 June 2016 with Roll of Attorneys No.
65124. A copy of the Certificate of Membership in the Philippine
Bar is hereto attached and marked as Annex “A”;
4. Petitioner is a member in good standing of the Integrated Bar of
the Philippines (IBP), Cebu Chapter, as evidenced by the IBP
Official Receipt with number 1002825 dated January 04, 2016 and
the Certification of Good Moral Character issued by two (2)
executive officers of the local chapter of the IBP, Certification from
the Criminal Cases Section of the Office of the Clerk of Court of
the Regional Trial Court, Cebu City and the Certification of Good
Moral Character and Membership in Good Standing issued by the
Office of the Bar Confidant of the Supreme Court of the
Philippines hereto attached as Annexes “B”, “C”, “D”, and “E”,
respectively, and made integral parts of this Petition;
5. Petitioner has been a resident in the Philippines for more than one
(1) year and presently working in Senining †, Belciña, Atup, Entise,
Limalima, Bantilan, Borgonia & Condat Law Offices which holds
office at ML Legal Division, M. Lhuillier Compound, 1082 J. Panis
St. (Gov. M. Cuenco Avenue) Kalubihan, Talamban, Cebu City,
where he may be contacted through telephone nos. (032) 354-
9767 / (032) 236-0962 / (032) 236-0963 and (0922) 883-6111;
6. As mandated by the pertinent rules, petitioner undertakes to
inform the Executive Judge of the Honorable Court of any change
in his office address;
7. Petitioner has paid his Professional tax for the current year and
was issued a Professional Tax Receipt (PTR) with numbers 861950
dated January 04, 2016. A copy of the PTR is hereto attached and
marked as Annex “F”;
8. Petitioner has never been convicted of any crime involving moral
turpitude; A copy of Petitioner’s NBI clearance is hereto attached
and marked as Annex “G”;
9. Petitioner now desires to be appointed and commissioned as
Notary Public acting for and in the City of Cebu, Philippines;
10. Petitioner is attaching as Annex “H” three (3) passport-size
colored photographs with light background, taken within 30 days
of this application, unretouched, with Petitioner’s name and
signature at the bottom part of the photographs;
11. Petitioner is attaching as Annex “H-1” a sheet of paper with three
(3) specimen of petitioner’s customary signature;
12. Petitioner has paid in full the proper filing fees, as evidenced by
the attached official receipt; A copy of aforementioned official
receipt is hereto attached and marked as Annex “I”;
13. Petitioner has not applied for a Notarial Commission in any other
court in the Philippines for the term applied for, and;
14. Petitioner possesses all the qualifications and none of the
disqualifications and desires to be appointed as a Notary Public
for the City of Cebu and the Municipalities within the jurisdiction
of this Honorable Court.
PRAYER
WHEREFORE, premises considered, it is most respectfully prayed of
this Honorable Court that Petitioner NESTOR CRISPIN MIGUEL B.
LUMAPAS be appointed as Notary Public for the City of Mandaue and the
Municipalities under the jurisdiction of the honourable court.
Other reliefs appurtenant thereto are likewise prayed for.
