Incident Report Roof Deck

During a routine inspection of Tower 2 of the Congressional Town Center project, it was discovered that an electrical conduit at the machine room roof deck had been damaged, likely by unknown individuals. This was causing water leakage into the machine room. To address the issue, the electrical contractor was notified and another PVC conduit was connected to block the water leakage and repair the problem. Pictures were taken to document the incident and repairs.

INCIDENT REPORT

PROJECT : Congressional Town Center Tower 2


LOCATION : 23 Congressional Ave. Brgy. Toro, Quezon City
DATE : September 25, 2023

Details of the Incident:

During the physical and visual inspection conducted last September 23, 2023. It is
found that the Electrical conduit PVC at roof deck of machine room was damaged and
cut by unknown people and finished the floor with water proofing; it is the cause of
water leakage at the machine room.

Actions taken:

1. The incident was reported immediately to the Electrical Contractor.


2. Inspected the area and traced the water leakage.
3. Connect another PVC for extension to block the water leakage.
