BARANGAY SAN JOSEL COUNCIL’S SUPPORT TO THE LEARNING CONTINUITY


PLAN OF SAN JOSE CENTRAL SCHOOL

This Memorandum of Understanding is entered into this 26th day of August, 2020 in San Jose
Central School, by and between:

The SAN RAFAEL ELEMENTARY SCHOOL with School Identification Number 121335, a
public elementary school, with its principal address at Brgy. San Rafael, Dulag, Leyte,
represented in this Agreement by its School Head, MA. CRISTINA A. SERVANDO, Filipino,
of legal age, hereinafter referred as the SCHOOL;
-and-
BRGY. COUNCIL OF BRGY. SAN RAFAEL, DULAG, LEYTE, a legal entity, operating
under the Local Government Code of the Republic of the Philippines as sanctioned by the Leyte,
represented in this Agreement by its Brgy. Chairman, HON. DOMINADOR AGULLO,
Filipino of legal age, hereinafter referred to as the STAKEHOLDER,

WITNESSETH:

WHEREAS, the SCHOOL is geared towards achieving quality education for all its learners,
whose standers of learners’ achievement, both academic and non-academic, are the central focus
of school accountability assessment.
WHEREAS, the SCHOOL, in its commitment to pursue education among its learners crafted
the SCHOOL EMERGENCY ROADMAP, a LEARNING CONTINUITY PLAN for School
Year 2020-2021, which encompasses the conceptualization and implementation of
contextualized mechanism in order to sustain the delivery of basic education service at San
Rafael Elementary School (SRES) amidst the pandemic and other health-related crisis;
WHEREAS, this noble aspiration is a collaborative effort of the school and its supportive
stakeholders who wholeheartedly accepted their respective accountabilities in this endeavor;
WHEREAS, the BRGY. COUNCIL OF BRGY. SAN RAFAEL, as STAKEHOLDER
acknowledges that it has accountability and wholehearted commitment to the school in
supporting all the programs and projects of the SCHOOL, as reflected in one of the objectives of
the council;
WHEREAS, the BRGY. COUNCIL OF BRGY. SAN RAFAEL shall serve as support group
and as a significant partner of the school whose relationship shall be defined by cooperative and
open dialogue to promote the improved performance of the students;
WHEREAS, the SCHOOL believed that its LEARNING CONTINUITY PLAN will be
fruitless and futile if it will not be implemented properly and without the support of its
stakeholders among which is the BRGY. COUNCIL OF BRGY. SAN RAFAEL;
WHEREAS, to achieve its objectives, the SCHOOL needs to into an agreement with the
BRGY. COUNCIL OF BRGY. SAN RAFAEL to seek support and commitment in the
school’s implementation of its various programs and projects;
WHEREAS, the BRGY. COUNCIL OF BRGY. SAN RAFAEL, being one of the school’s
major stakeholder expressed its commitment to be accountable in helping the SCHOOL achieve
its goals as stated above;
WHEREAS, the SCHOOL and the BRGY. COUNCIL OF BRGY, SAN RAFAEL,
hereinafter collectively referred to as “PARTIES” undertake a collective agreement for the
successful implementation of various programs and projects of the school;
NOW, THEREFORE, for and in consideration of the foregoing premises, the PARTIES
hereby agree as follows;
1. That the PARTIES will be accountable in building a strong and sustainable partnership
with each other in successfully carrying out its LEARNING CONTINUITY PLAN;
2. That the BRGY, COUNCIL OF BRGY. SAN RAFAEL will help in the provision of
A4 size Bond Papers for the reproduction of worksheets/learning activity sheets to all
students;
3. That the BRGY, COUNCIL OF BRGY. SAN RAFAEL will support the school’s
initiatives in the distribution and retrieval of worksheets/learning activity sheets from all
learners within the Barangay;
4. That the BRGY, COUNCIL OF BRGY. SAN RAFAEL will assist the school in the
generation of resources to sustain school’s program and projects;
5. That SCHOOL will closely implement and monitor the implementation of its
LEARNING CONTINUITY PLAN;
6. That the SCHOOL will give recognition to the STAKEHOLDER for their support to
achievement of school’s program and project during their annual Alumni Homecoming,
Recognition Day, or Graduation Day.

4. That the SCHOOL will give recognition to the STAKEHOLDER for their support to

school’s initiatives in the distribution and retrieval of worksheets/learning activity sheets
from the learners;
4. That the SRES GENERAL PARENTS-TEACHERS ASSOCIATION will support
and guide the learners in the modular learning instruction;
5. That the SRES GENERAL PARENTS-TEACHERS ASSOCIATION will assist the
school in the generation of resources to sustain school’s program and projects;
6. That the SCHOOL will closely implement and monitor the implementation of its
LEARNING CONTINUITY PLAN;
7. That the SCHOOL will give recognition to the STAKEHOLDER for their support to
achievement of school’s program and project during their annual Alumni Homecoming,
Recognition Day, or Graduation Day.
