1. INTRODUCTION
1.1. Background
Construction of 2-webbed Pre-stressed Concrete Bridge with open foundation is a bridge
project on Hariya Khola, Udayapur which connect Devidhap village and Hadaiya village.
It is a major bridge of the Siddhicharan National Highway. This project is the sub project
of Department of Road and implemented by Division Road Office Lahan.
This project plays vital role on economic development of Udaypur district through
connectivity and twelve month transportation facility. About 50-60 thousand populations
are projected to benefit by the project. This bridge is 4-span bridge each of 40m length
with total span of 160m. The project location is on Hadaiya khola, Udayapur. Company
has already mobilized key personnel and construction equipment at site. Site office has
established on site and company has started to collect the construction material.
1.2. Objectives
The main objective of preparing quality assurance plan is to complete all the items of
works (methods, procedure, organization for the quality control, the sequence of
testing) under the contractual obligation within the time frame such that the works
executed should meet or exceed the desired performance criteria, quality standards
(specified by drawings & technical specification which are the part of the Contract
Documents).
2. QUALITY ASSURANCE
2.1. Introduction
Planning, monitoring and controlling standards for quality are fundamental in both the
design and construction phases of a project. This aspect of a project, while closely
interrelated with cost, schedule, procurement and value engineering deserves its own
amplification.
The focus of the document is about the Quality Assurance Planning. It will describe the
different quality assurance steps for achieving the required quality standards. Then there will
be the brief working procedures and methodology of different items of works, testing
procedures and descriptions, quality standards and quality controls. Allocation of
responsibilities is equally important for attaining required quality standards and hence it
describes the responsibility of different key personnel and organizational breakdown
structures near the end for clear understanding of line of command or flow of information. A
brief description of inspection and testing to be performed presented at the end will be
valuable controls for assuring required quality standards.
2.2. Definitions
Quality Assurance plan is a plan to be made prior to start any manufacturing/executing
process to assure about quality of works to achieve its final goal. It can be used as a device to
control the process. A good quality of works could be achieved by proper selection of
construction materials, good workmanships and supervision and also maintaining proper
quality control system after construction. It is our ambition and responsibility to execute
quality works only. So, that we have already employed below listed personnel’s and set their
responsibilities to achieve our final goal.
It involves the use of all standards and procedures to ensure that a product or a facility
meets or exceeds desired performance criteria. It also usually includes the
documentation necessary to verify that all steps in the procedures have been
satisfactorily completed.
Quality assurance is all the planned and systematic activities implemented within the
quality system to provide confidence that the project will satisfy the relevant quality
standards and it should be provided through out the project.
2.3. Quality Manual
There is no such quality manual; however a brief and concise description of quality
requirement and quality procedure has been developed in accordance with the
specifications.
2.4. Drawings and Specifications
Drawings and Technical Specifications of the Contract or as approved and produced by
DOR are the part of the contract documents. These are the main documents that show
clearly the quality requirement of the employer and hence will be strictly followed
during construction with some specification if required by the Employer.
Note:
The Technical Specifications of the Contract is taken as the Standard Specifications
for Works published by DOR and the Contract Agreement Document as mentioned in
Volume V of General Conditions of Contract (Quality Contral). & as mention in
Section VI Specification.
2.5. Work Procedure
1. General Excavation of abutment under BOQ Item no: 2.1
1. Construction survey layout for locating the abutment- Joint survey
2. Preparation and approval of the working drawings.
3. Layout in the field.
4. Check and verify the pile location by the engineer.
5. Excavation of bore hole for pile through boring machine.
6. Transportation of the excavated material to the environmentally safe tipping
site.
7. Check cut line, levels and slopes as per drawings.
8. If ok, excavation is completed otherwise make correction in accordance with the
design and drawings.
2. Rebar Works in pile foundation under BOQ Item no: 2.6
1. Detail study of design and drawing to understand completely about size and
length of the rebar.
2. Prepare and approved the bar bending schedule by project engineer.
3. Lab test of rebar and get approval by project engineer.
4. Transport the approved rebar to site and placed in safe site.
5. Bending of rebar to make straight and cutting the rebar in approved size as per
BBS and molding it in approved shape and size.
6. Preparation of rebar in pile shape and check and verified by site engineer before
placing.

3. Formwork for Concreting Works BOQ Item no: 2.5
1. Identification of location and layout the types and size of form works to be
carried out for Item under BOQ.
2. Prepare and make approval of dimensions for design and drawings for the
same,
3. Fixation of line and level as required by the structured for casting defining
quantities as defined in the items of BOQ of the Contract documents and as
defined in technical specifications of the contract.
4. 2 m height staining is built for first stage.
5. Height of the staining is calibrated and approved by site representative.
6. Damage portion of staining at top of previous stage is properly repaired before
constructing the next stage.
7. Checking line level and centering of shuttering or form works before pouring
the concrete as per design and drawings approved by the engineer.
8. Get approval from the engineer representative for the completed section before
pouring concrete.
4. M25 RCC Works in foundation under BOQ Item no: 2.4
1. Test of Course aggregate, fine aggregate and cement is carried out before
transporting the material at site.
2. Transporting and storing the material safely at site.
3. Preparing the staining and get approval from site engineer.
4. Preparing the concrete mix through mixture in approved ratio.
5. Testing the slumps of concrete mix and approved by engineer.
6. Preparation of cubes samples for testing of compressive strength of concrete
7. Placing the concrete through concrete pump.
8. Compaction of concrete is ensuring.
9. Curing the concrete for at least 7 days.
10. Final Measurement of concrete works is taken in presence of office
representative.
5. Formwork for Concreting Works BOQ Item no: 3.1 & 4.3 & 4.4
1. Formwork is done in perfect line and level and maintaining the specified cover
for rebar.
2. Scaffolding of formwork tightly through iron pipes.
3. Cleaning the debris if present and ensure clean before concrete start.
4. Check the line and level and get approval of site engineer before concrete
placing.
5. Removal the formwork after 14 days of complete curing and place the
formwork in safe site.
6. Rebar Works in Sub structure under BOQ Item no: 3.3 & 4.5
1. Detail study of design and drawing to understand completely about size and
length of the rebar.
2. Prepare and approved the bar bending schedule by project engineer.
3. Lab test of rebar and get approval by project engineer.
4. Transport the approved rebar to site and placed in safe site.
5. Bending of rebar to make straight and cutting the rebar in approved size as per
BBS and molding it in approved shape and size.
6. Preparation of rebar in pile shape and check and verified by site engineer before
placing.
7. Placing of rebar in proper line and level on approved formwork.
8. Maintain the specified cover through cover block.
9. Check and verify the rebar work and get approval before concreting.
10. Final measurement of rebar works is taken jointly with office representative.
7. M25 RCC Works in sub- structure under BOQ Item no: 3.1
1. Test of Course aggregate, fine aggregate and cement is carried out before
transporting the material at site.
2. Transporting and storing the material safely at site.
3. Check and approved rebar and formwork before concrete placing by site
engineer.
4. Preparing the concrete mix through mixture in approved ratio.
5. Testing the slumps of concrete mix and approved by engineer.
6. Preparation of cubes samples for testing of compressive strength of concrete
7. Placing the concrete through wheel barrow.

8. M35 RCC Works in sub- structure under BOQ Item no: 4.1

2. Design mix is done on laboratory and gets approval from project manager.
3. Transporting and storing the material safely at site.
4. Check and approved rebar and formwork before concrete placing by site
engineer.
5. Preparing the concrete mix through mixture in approved ratio obtain from
design mix.
6. Testing the slumps of concrete mix and approved by engineer.
7. Preparation of cubes samples for testing of compressive strength of concrete
8. Placing the concrete through wheel barrow.

9. M45 RCC Works in sub- structure under BOQ Item no: 4.2

4. Check and approved rebar and formwork before concrete placing by site

10. Stone Masonry Works under BOQ Item no:3.7
1. Identification of location to layout the types and size of structures to be
constructed.
2. Prepare and make approval of the drawings for the same.
3. Layout of structure as per working drawing.
4. Carry out the earthwork in foundation to the required depth and grade and cross
section will be measured for excavation.
5. Approval of the material sand, stone boulder, water and cement will be carried
out prior to the transportation of the materials to site.
6. Material from excavation is disposed off to the spoil areas or if material is
appropriate for use it will be used for the formation of road embankment or
backfilling or dykes etc as directed by the engineer.
7. Compaction as required is given to the finished foundation bed of structures.
8. Construction work will be carried out as per the design and drawing approved
by the engineers and the procedure for the work will be done as specified by
Specification of the Contract or as instructed by the engineer.
9. Sampling of cement sand mortar cube will be carried to the laboratory for
necessary strength testing.
10. Curing will be carried out for at least 7 days.
11. Back filling and compaction works with approved materials by the engineers
will be carried to the structures and density test will be carried out.
12. All the waste material left after the completion of the section will be transported
to other construction sites and necessary location for further use and all those
unnecessary material will be disposed to the defined spoil areas and the road
will be made clear for project’s , adjacent contractors , local and public
vehicular movement.
13. Acceptance and approval of work executed by the engineer.
11. Sub-grade Preparation under BOQ Item no: 5.3
1. Survey work and layout the road as per design drawing.
2. Grading, leveling and filling as required by the design and drawings approved
by the engineer to the full width of the road.
3. Take sub grade sample for the determination of CBR/MDD aomd OMC
4. Watering and Compaction of the sub-grade of road bed will be carried out to
meet the required field density as specified in the Technical Specifications of
the Contract.
5. Compaction with watering by vibrating roller until the required field density and
smoothness will not be achieved.
6. Checking line level, geometric design and field density of the finished surface.
7. Get approval from the engineer representative for the completed section.
12. Sub-Base Laying Works under BOQ Item no: 5.4
1. Identification of potential quarry

13. Crusher Run Sub base Course(Improved sub base)Laying Works under

finished surface.
17. Get final approval from the engineer representative for the completed
section.
14. Bituminous Carpeting Works under BOQ Item no: 5.6,5.7 & 5.8
1. Test of bitumen and its approval.
2. Preliminary test of quarry material
3. Approval of quarry material
4. Production of Carpeting aggregate.
5. Production control test.
6. Material stockpiling in acceptable shape, size and quantity.
7. Suitability test.
8. Approval of stockpiled Carpeting aggregate.
9. Select the length of Bituminous Carpeting and detailed the preliminary
work.
10. Traffic control to the site by warning sign, control barriers, traffic cones etc.
11. Cleaning of existing surface containing dust/dirt/cow dung if any
12. All the necessary equipment will be kept stand by at site for operations such
equipment are Bitumen Distributor, Pneumatic Tyred Roller, Chips
spreader, Tippers and Trucks, Bitumen spray rate checking tray, Bitumen
Decanter, Bitumen, Kerosene, Fire extinguisher, Fist Aid Box, Service
Vehicle etc.
13. The entire stand by equipment will be checked and run thoroughly to
guarantee the operations.
14. Carry out site trials as instructed by lab.
15. Spraying of specified bitumen at specified rate for prime coat.
16. Spraying of specified bitumen at specified rate for Tack coat.
17. Spreading of approved sand aggregate using spreader.
18. Burning the approved aggregate for carpeting in burner to the specified
temperature.
19. Transporting it to the site & placing & laying it to specified thickness &
width as per engineers' instruction.
20. Check the aggregate has been placed correctly and the workforce are ready
to spread the aggregate after the chips spreader.
21. Check equipment for rolling is positioned ready for rolling. It is
recommended the distributor makes a spray run of 100 meters to allow for
immediate covering of the binder with aggregate.
22. 12 ton pneumatic-tyred rollers will be used in the construction of Premixed
carpet. The section of treated road will be reopened to traffic after 3 passes
of the rollers but on the day of construction, the surface treated will receive
a minimum of 15 passes.
23. Sand sill is provided to premixed carpet surface 7 compacted by roller.
24. Speed of traffic will be limit as instructed by engineer for two to three
weeks after construction to minimize the hazard of loose excess aggregate
being thrown up into the air.
25. A minimum period of 8 weeks will be elapse between the construction of
the first and the second layers. For second layer same process will be
followed as first layers.
26. Get final approval from the engineer representative for the completed
section.
15. Backfilling Works under BOQ Item no:3.4 & 3.5
1. Testing of Filling Materials
2. Collection & transportation of Backfilling materials on site.
3. Filling the trench, foundation in different layer & compacting it by
providing necessary watering.
4. Approved it by engineer/ engineer's representative.
16. Dry Stone Soling Works under BOQ Item no:3.6

17. Pre-stress works under BOQ Item no: 4.6,4.7 & 4.8
1. Full detail of plant used for installation of pre-stressing tendons, material
proposed to used and arrangements proposes to used for pre-stressing is
submitted to project engineer and get approved.
2. All pre-stressing components, sheeting ducts, anchorage, pre- stressing
tendon jack etc are stored in clean and dry place.
3. Ensure the material is free from rust.
4. 53 grade OPC cement is used for pre stressed concrete.
5. Lab test of tendon wire is done for each role in lab in presence of office
representative.
6. Tendon wire is clean with suitable solvent solutions.
7. Packing of pre-stressing strand is removed just prior to making of cable for
placement.
8. Suitable stands are provided to facilate uncoiling of wires/ strands without
damage to steel.
9. Suitable arrangement is made to avoid the possibility of steel coming into
contact with ground.
10. Correct functioning of the jack, pump and leads is checked.
11. Cable is de-tension. Slide it in its duck to check that it is not blocked by
mortar which has through holes in the sheath.
12. Modulus of elasticity of steel for the particular lot is checked and approved
from laboratory.
13. Cutting and sealing work is done after approval of project engineer.
14. Complete record of pre-stressing operations along with elongation and jack
pressure data shall be maintained in approved format.
15. Pre-stressing and grouting is done in 2 days.
16. Admixture is finalized and approved from engineer before grouting is done.
17. Grouting is done by using grout machine.
18 Gabion Works BOQ Item no: 17
1. Identification of location to layout the types and size of structures to be constructed.
2. Prepare and make approval of the drawings for the same.
3. Approval of the material; gabion wires – tested, stone boulder will be carried out prior to
the transportation of the materials to site.
4. Production of Gabion boxes to the required sizes and transportation of this and boulder
to the required quantities.
5. Layout of structure as per working drawing.
6. Carry out the earthwork in foundation to the required depth and cross section will be
measured for excavation.
7. Material from excavation is disposed off to the spoil areas or if material is appropriate
for use it will be used for the formation of road embankment or backfilling as directed by
the engineer.
8. Compaction as required is given to the finished foundation bed of structures.
9. Construction work (i.e. filling of stone in the boxes of gabion and weaving of box) will
be carried out as per the design and drawing approved by the engineers and the
procedure for the work will be done as specified by Specification of the Contract or as
instructed by the engineer.
10. Checking of final level of the structure.
11. Back filling and compaction works with materials approved by the engineers will be
carried to the structures. Density test for the same will be carried out after compaction.
12. All the waste material left after the completion of the section will be transported to other
construction sites and necessary location for further use and all those unnecessary
material will be disposed to the defined spoil areas and the road will be made clear for
project’s , adjacent contractors , local and public vehicular movement.
13. Acceptance and approval of work executed by the engineer.
2.6. Schedules
2.6.1. Project Work Schedule
Contractor work schedule submitted during the contract signing is revised and attached
here in Annex-1. This schedule will be used in planning the procurement of materials,
equipments and will be revised as per the contract requirement.
2.6.2. Material Management Schedule
Based on the project work schedule material management schedule is presented in
Annex. This show the quantity of different material required to performed the
construction works as per the presented schedule. Material management schedule is
attached in Annex -B
2.6.3. Material Testing Schedule
Based on the detailed work Program the material testing Schedule will be prepared and
the frequency of test has been prepared and included in the Appendix-3. Standard forms
and formats provided by the project will be used for keeping various types of test records
and if such formats are not available contractor will develop the format and get approval
from the engineer
2.7. Organization (Rajendra/Jagadamba/Galwa JV Kathmandu)
2.7.1. Key Personals
Human resources are the key factor for searching, monitoring and controlling other
activities and resources in any industry or in any business community. Properly
organized and managed human resources produce the best output to achieve the pre-
determined goal.
Our organization Rajendra/ Jagadamba/ Galwa Jv for the construction of 2-webbed
Pre-stressed Concrete bridge with open foundation contract no. DROLHN/337320-
4/072/073-30 is focus to achieve our pre-determined goal under our contractual
obligations .
Our team will fully cooperate with Employer and the Consulting Engineers’ Team
of DOR and with professional spirit and workmanship in accordance with the
Technical Specifications of the Contract and other general public rules and
regulations of the Nation.
With full team spirit our team is mobilized to execute the works under our contract
and try our level best to complete our contractual obligations within the stipulated
time.

2.7.2. Allocation of Responsibilities

2.8. Monitoring of QAP
For the monitoring of the QAP contractor will carry out the following activity
A monitoring unit chaired by Contract Manager followed by the engineer/site
engineer/overseer/Lab technician/surveyorswill be formed and meeting will
be conducted on regular basis for the assessment of the quality of materials
and quality of works executed and step to be taken for the for the further
improvement as required will be finalized and circulated among the staff

3. Sources of Construction Materials
3.1. Extraction of Material from Quarry
The main sources (Quarry Sites) of the construction materials (Aggregates, boulders,
sand) are presented in tabular form (Annex –D) indicating expected quantity and type of
material to be extracted. Other materials like bitumen, cement, and gabion will be
purchased from the market and tested before use and approved by the Engineer. The
approval of quarry sites will be taken from the Engineer before its operation following
the guidelines stated in the Contract Documents.
3.2. Reinstatement of Quarry Site
All quarry sites shall be made stable after the closure of the sites. A quarry management
and restoration plan will be submitted during the quarry approval showing the work to be
done for the restoration. These works consist of trimming the slope to stable condition,
filling the excavated portion with suitable materials, providing breast walls, retaining
walls, or by Bioengineering. Type and quantity of structure will be decided after the
completion of extraction depending upon degree of damage to the site and will be
approved from the engineer before the execution.
After the reinstatement of the sites, the Contractor will invite the Engineer for the
inspection. The Contractor will be released from his obligation only after the Engineer
satisfies with the reinstatement activity and ensures that the sites are not vulnerable from
any further damages.
4. Dumping of Excess Material
The unsuitable / surplus materials shall be properly managed and dumped at the approved
locations. The Contractor will propose the Spoil dumping sites and after getting approval
from the Engineer, these sites will be used for dumping unsuitable / unused materials.
Location of these sites is shown in Appendix –E. These proposed sites may change
during the construction and will be informed to the engineer if so and get approval of the
new dumping site.
5. Amendment to quality Plan
Any confusion or dispute in the Quality Assurance Plan (QAP) will be discussed on the
regular monthly meeting with Project Engineer/ Consultant and will be resolved based on
the technical specification.
6. Summary
Quality of product in any construction industry is meant the fulfillment of the
requirements of the employer described by drawings and specification which are the part
of Contract Documents.
Our company will ensure adequate supervision and quality control of works and material
at all times by providing well experiences technical team, defining the responsibilities of
the key staffs, providing precise instrument / equipment and setting out working
guidelines as required by quality standards. Testing of material will be done on Central
Laboratory of DOR and Compressive strength test of Concrete cube will be done on
Division Road Office Lahan laboratory.
