Thank you for seeing Mrs Sharma, a 60- year old retired woman. Your further management to control her
blood glucose level would be highly appreciated.

Mrs Sharma has a medical history of NIDDM for 20 years. Metformin and glipizide has been used to control Commented [JG1]: have
blood sugar level. She is known to be allergic to pencillin. Please note that she has strong family history of Commented [JG2]: her
diabetes mellitus. Commented [JG3]: penicillin

On 29/12/2013, Mrs Sharma attended the health centre to check her blood glucose level. Recent glucose
level was high about 6 to 18 mmol/l. Her blood pressure was 155/100. Her medical condition regarding eye
examination, weight, bowel and nutrition was good. Some tests were required and Atacand was added.

2 weeks later, pathology report revealed poor control of blood sugar proved by high HbA1c and high
cholesterol. Other laboratory results were normal. Accordingly, atorvastatin 20 mg was added and
metformin dose changed from 2 nocte to 1 bd. At the end of that month, blood pressure and glucose level
improved.

Today, lipid profile showed improvement as the cholesterol decreased from 6.2 to 3.2, LDLC 1.1 and
triglycerides decreased from 2.4 to 1.7.

In view of the above, I am referring Mrs Sharma for your further assessment and control of her diabetes.
