1.1. Some cultures held in the PNCM are not preserved in liquid-dried form, i.e., non-spore forming multicellular
fungi, hence, may only be released in active form.
1.2. Active culture rates will apply to these requests (10% on top of preserved rates).
1.3. Preparation of active cultures may take longer depending on the microorganism being requested and the
stocks that we have of the said microorganism. Some microorganisms grow slowly, as such may require
more time to prepare. The release schedule was adjusted following current work arrangements in
response to the pandemic.
1.4. Prices may change without prior notice.
2. Requirements for Requesting Cultures
2.1. The end user should have been properly trained in handling and disposal of microbial cultures,
and the laboratory have the equipment for proper handling of the culture(s) requested and the
laboratory have the equipment for proper handling of the culture(s) requested. These should be
explicitly indicated in the request letter of the adviser of the student.
2.2. The required documents (letter of request, MTA, AOR, EUC and copy of valid Government-issued
IDs) must be submitted as single file in PDF format to the PNCM official e-mail address.

2.2.1.1. Address the letter of request to the Director of BIOTECH, cc the Head Curator of
PNCM. Indicate in the letter the following:
2.2.1.2. the microorganism/s requested,
2.2.1.3. the purpose for requesting the microorganism/s,
2.2.1.4. the requestor’s or supervisor’s CAPABILITY to handle the
microorganism and if laboratory to be used is properly equipped with
a Class 2 biohazard safety cabinet particularly for biosafety level II
organisms such as Salmonella and if cultures will be handled by a
microbiologist, and/or if will be used under the supervision of a trained

2.2.1.5. FOR PROFESSIONALS FROM THE ACADEME,
GOVERNMENT/PRIVATE INSTITUTION/COMPANY, the letter
written by the staff should be noted by the person with the next higher
rank of the institution/company.
2.2.1.6. FOR COLLEGE AND POST GRADUATE STUDENTS, the letter of
request should be written and signed by the student’s adviser (or a
highly trained individual who is ultimately responsible for supervising
the student’s research) on the student’s behalf indicating everything
mentioned in 2.1.1.1, 2.1.1.2, and 2.1.1.3. The letter written by the
adviser should be noted by the person with the next higher rank e.g.
Department Chair, Dean, Director, Principal and etc. STUDENTS
should not write the letter. Letters written and signed by students will
not be accepted.
2.2.1.7. HIGH SCHOOL STUDENTS will only be allowed to request for GRAS
(generally recognized as safe) microorganisms. For letter
requirement, see 2.2.1.6).
2.2.2. FORMS

2.2.2.2. Material Transfer Agreement (MTA) form (PNCM Form FS-17) duly accomplished
and signed by the end recipient. Students must let their adviser (or a highly trained
individual who is ultimately responsible for supervising the student’s research) fill out
and sign the said form. Please fill-out only ITEMS 2, 3, and 5.
2.2.2.3. Acceptance of Responsibility form (PNCM Form FS-18) duly accomplished and
signed. For professionals, the “Authorized Representative” may be company technical
analysts or their supervisors or company owners who are also the end recipient
signatory in 2.1.2. The scientist field may be filled out by the said technical analysts or
company or non-company consultants who are experts in the field.
Students must let their adviser and a highly trained individual who is ultimately
responsible for supervising the student’s research fill out and sign the said form. The
adviser should fill out both the “Authorized Representative” and “Scientist” fields in the
absence of other supervising individuals. However, if a highly trained individual will be
supervising the student, e.g. a DOST Microbiologist, etc. the said individual should fill
out the “Scientist” field and the adviser should fill out the “Authorized Representative”
field. The adviser must be knowledgeable in microbiology and microbiological
techniques.
2.2.2.4. End-Use Certificate (EUC) (PNCM Form FS-19) duly accomplished and signed by
the head/authorized person of the company/organization. A filled-out example may be
requested as reference.
2.2.3. GOVERNMENT IDENTIFICATION CARDS OF SIGNATORIES
2.2.3.1. A scanned copy of at least one of the following valid or Government-issued ID of all
signatories:

2.3. The full payment for the cultures. A customer may pay in cash in person or may send the payment via
LBC Pesopak (Door-to-Door Delivery) or any door-to-door delivery service. SEND PAYMENT TO:

3. Processing of Requests and Culture Release
3.1. The processing of a customer’s request shall commence ONLY UPON RECEIPT OF ALL THE
REQUIREMENTS (2.1 and 2.2) and will follow the processing schedule as in section 1 (release schedule).
An e-mail will be sent for confirmation of receipt and approval of request.
3.2. A customer may choose to personally pick up the cultures in person once they are ready. However, this
is NOT an option for HS students as cultures will be sent directly to their adviser.
3.3. The customer may also opt for the cultures to be shipped to their desired addresses (as indicated in
EUC). This option will require a courier fee for every 5 cultures. As shipping prices are subject to change
without prior notice, we will follow the current shipping rates of courier companies. We may require
additional courier fee if the requested active cultures cannot be released in one batch due to viability
issues, e.g. Vibrio spp. (can only be stored for a week) and sporulating multicellular fungi (can be stored
for a month or more).
3.4. The PNCM preferably ships cultures via LBC. One may consult LBC or other shipping companies for their
effective shipping rates.
3.5. The courier fee should not be included in the cheque payment and must be sent separately via LBC
Pesopak (Door-to-door delivery) or any door-to-door delivery service.
3.6. Official receipts will be sent together with the culture/s.
4. Denial of Requests
4.1. The PNCM reserves the right to deny requests from parties for any reasons deemed fit.
4.2. Affected parties will be e-mailed after a review of the requirements.
5. Refunds
5.1. Payments from requests that were denied will be sent back to the buyer (as indicated in EUC 1.b and
1.c). The courier fee for this will be deducted from the payment made. However, the client may claim
the payment personally.
5.2. For cultures that cannot be revived from stocks, the PNCM will inform the customer and refund the
amount of each culture. In this case, the courier fee will be shouldered by the PNCM.
5.3. Requests that have been approved and paid are no longer subject to refund. There can be no
cancellation of requests once approved.

and the contributor (depositor) indicated by the PNCM as the source of the Material in all
publications that refers to the Material.
d. The PNCM only warrants for the viability of the Material, and only for a period of 30
days upon shipment. If otherwise, the PNCM has the option to either refund the fee paid
to the PNCM for such Material (exclusive of shipping and handling charges), or, replace
the Material. The Recipient must report the lack of viability to the PNCM in writing within
the warranty period. Except as expressly provided above, the PNCM gives no
guarantee, express or implied, for the material, including merchantability, or fitness
for a particular purpose.
e. The PNCM makes no representation that the use of the Material will not infringe any patent
or other proprietary right. The Recipient will hold the PNCM and its staff harmless from
any claims or liabilities that might arise as a result of the use of the Material by the
Recipient.
