RESPONSE

1.0 PURPOSE
To identify the potential emergency situation relevant to company operation and the response
to the emergency situation for preventing and mitigate the likely illness and injury and
damage to environment associate to the emergency situation.

2.0 SCOPE

This procedure covered from identification of potential emergency situation until emergency
investigation and reporting.

3.0 REFERENCE
IMS Manual IM-01
Guidelines on First-Aid Facilities in the Workplace, DOSH
Factories and Machinery Act 1967 (Act 139), Section 25
Machinery (Safety, Health and Welfare) Regulations 1970, Regulation 38
Occupational Safety and Health Act 1994 (Act 514), Section 15(1)

4.0 DEFINITION / ABBREVIATION

7.1 Identification of Potential Emergency Situation


The relevant potential emergency situations have been identified as below;
a) Fire Accident
b) Major Incident/Accident
c) Chemical Spillage
d) Explosion

7.2 Set up the ERT


a) HSEC shall establish the Emergency Response Team (ERT) to respond
to emergencies and to attempt to bring the situation under control.

b) ERT is also to assist the management in carrying out a certain safety


management program for the company.

c) ERT shall comprise of employees from various departments, who have


knowledge or have been trained in responding to emergencies. The
team shall have a team leader.

d) ERT organization chart, role & responsibilities are defined in


Attachment 1.

e) ERT member will go through drills to practice the skills they acquired
during training.

7.3 Roles & Responsibilities


a) As per Attachment 1

7.4 First-Aid Box


a) First-aid boxes should be made of sturdy material and be portable so
that it can be taken to the site of an incident. The boxes should also be
clearly marked with a green crescent on a white background.

b) Each first-aid box shall be placed in a clearly identifiable, well


illuminated and accessible location.

c) The box shall be kept locked and the key thereto kept by a responsible
person available during all working hours. The employees should be
informed of the location of all first-aid boxes.

d) First-aid boxes shall contain a sufficient quantity of suitable first-aid


materials (refer to First Aid Box Inspection Check List).
a) First-aid boxes be checked on monthly basis so as to make sure they
are fully equipped and all items are usable (refer to First Aid Box
Inspection Check List).

7.5 Fire Extinguisher


a)

7.6 Training & Drills


b) HSEC shall identify & organize a training/briefing on the emergency
response and preparedness for the ERT members, chemical handler
and other emergency responder to enhance their duty performance.

c) Besides the ERT members, all employees shall also be given


emergency response training.

d) The drills shall be conducted at least once a year to ensure all the
procedures are followed correctly.

e) All drills shall be recorded and reviewed the effectiveness of the


procedure, function or equipment, mobility of ERT and the duration of
the evacuation.

f) If required, the drills shall be alerted to the Fire & Rescue authority on
the fire drills.

g) Any special problem encountered should be noted together with the


corrective actions.

7.7 Emergency Response Plan (ERP)


a) HSEC and the ERT Leader who has the knowledge of the site
conditions and local regulations shall prepare the ERP for all
emergency situations identified.

b) The Emergency Response Team (ERT) may be involved in the


preparation of the plan or at least in the continual improvement of the
plan over time.

c) All employees shall be given emergency response training.

d) ERP will be amended when important components become outdated or


business and regulatory changes occur. The ERP shall be up-dated if
significant changes have occurred

7.8 Emergency Contact Number


a) Emergency Contact Number shall be maintained for the contact
numbers of the ERT (during work hours) or the relevant personnel
(during off-work hours) and the external rescue team e.g. Fire & Safety
Rescue, ambulance, etc.)

7.9 Evacuation Plan and Assembly Points


a) ERT shall prepare the evacuation plan. The evacuation plan or route
shall be updated promptly whenever there are changes in the premise
affecting the evacuation route.

b) The evacuation route shall be displayed in the necessary area. All exit
doors must be labelled.

c) The evacuation plan shall show the closest exits, backup exits, location
of the fire extinguisher, eyewash / toilets, PPE, chemical spill cleanup
supplies, first aid supplies, assembly points and other key elements.

d) All employees must know the primary exit route in case an emergency
occurs.

7.10 Evacuation Procedure


a) In the case of emergency, employees must walk calmly to the nearest
exit as described in the evacuation route. Exit the building and report
to the assembly area. Do not RUN but WALK calm and orderly.

b) Note: In case the gazetted exit route is blocked or affected by the


emergency; use the nearest exit route (alternative), otherwise; follow
the ERT's instruction.

c) Employees shall leave the rescue for those who are trained. It is not
necessary to try to help the ERT or stop to ask questions during the
evacuation process

d) For those who are away from the work area (either at training,
restroom, canteen etc.) follow the evacuation route posted for the area.
It is imperative to inform the ERT member of your whereabouts to
avoid problems during the roll call.

e) Employees shall gather at the assembly point and wait for further
instruction.

f) Unless instructed otherwise, after the ALL CLEAR signal by the


authority; all employees shall return to their department calmly and
orderly; to resume work.

g) Evacuation plan or route shall be updated promptly whenever there are


changes in the premise affecting the evacuation route. The evacuation
route shall be displayed in the necessary area. All exit doors must be
labelled.

h) Communication during an emergency shall be either through telephone


or P.A. System or whistles or fire alarm.

i) HSEC/Designated charge shall set their On-Shift Emergency Response


Team into motion and shall also summon additional help from other
members of the staff, as deemed necessary to response to the
emergency situation beyond normal hours.

1.2 Investigation & Reporting


a) For any HSE incident; ERT shall investigate by using the
Accident/Incident Investigation Report as soon as practicable, and in
any case within 24 hours after standing down and submit a report to
the HSER.

b) HSEC and ERT leader shall determine the adequacy of the corrective
action taken.

c) Respective emergency response plan and scenario shall be reviewed


for suitability and effectiveness by ERT.
