1. Are all support given to the shuttering for slab of adequate strength
and properly fixed and braced

2. Are all Props rests on the ground firmly?

3. Are all Props pin in place

4. Are all Props braced with ledger with pipe and tied by coupler?

6. Are bottom of props not on the any slope?

7. Are all legs of tripod on the firm ground?

8. Are wall tie pins in place correctly and well in position?

9. Is there provision of usage wooden pieces, stone pieces etc as distance

11 Are an Adequate illumination has been ensured?

B ABOVE THE FLOOR

11. Are edges of the slab properly barricaded?

12. Has vertical Rebar (20mm) been inserted at every 2mtrs minimum for
edge protection?

13. Are all loose materials removed from the slab?

14. Do all props start from the next lower slab for the Edge protection?

15. Is condition of cable of vibrator to be used while casting good


connected to the ELCB?

16. Are there any damage/ joint in the cable?

17. Are the safety belt, Gumboot, etc available for the workers engaged in
the casting?
18. Is lighting arrangement proper?

19. Are walkway challis provided to walk on reinforcement while casting


good?

C SURROUNDING AREA

20. Is approach to the Slab unobstructed and proper?

21. Is the width of a working platform properly maintained according to


usage?

22. Are opening in working platform kept safely covered/ fenced?

23. Is there a provision of handrails?

24. Is there a provision of anchoring full body harness?

25. Are all the workers have been explained safe work procedures?

26. Are ladder providing access to the Slab securely fixed top and bottom?

27. Are all the materials stored on the working platform secured?

28. Are walkway/ aisles and workplaces cleared of loose materials

29. Are route of the Concrete Pipe line clear, Is it supported at enough
places using proper stands, and Is it properly clamped?

30. Is Walky-Talky available for the communication between Pump


Operator and person on the Slab/

32. Is good approach for the transit miller to the concrete pump proper?

33. Is ball catcher available to clear the blocked line.

34. Is the stopper for the ball there in the place?

35. Are work area inspected prior starting to the job?

36. Are safety nets provided wherever required
