Ravi, an IT staff member, is working one evening and gets a call from Eric, a nurse on the
neuro intensive care unit. Eric cannot log into his EHR account to view his patient’s data. His
patient is not doing well, and Eric is very concerned that he needs to take prompt action to help
him, but can’t get to the patient’s information, which he needs to help him create his care plan.
He is sure he has not forgotten his password and is increasingly anxious.
Ravi checks with the IT security officer and together they quickly determine that someone tried
to access the EHR yesterday using Eric’s ID and password. The potential hacker went over the
6 limit of unsuccessful login tries. Ravi and the security officer also check with the human
resources department to make sure Eric is still an authorized employee. A human resources staff
member confirms that he is.
Ravi calls Eric back and asks if he knows anything about the attempts to access his account.
Eric responds that he doesn’t know anything about that and tells Ravi that he just got back from
vacation late last night. Ravi and the security officer then follow the organization’s security
policy, creating a temporary EHR account for Eric to use during his shift and leaving his
existing account intact for the security team to investigate. They also follow the policy by
writing up their findings and reporting them immediately to those responsible for investigating
attempted break-ins.
Discussion Question:
What would be the implications to patient data security and the organization if Ravi had not

Activity Title: Verbal and Nonverbal Communication
Instructions:
Case Study
A patient presented to his primary care physician (PCP) for the treatment of headaches and
episodes of altered consciousness. The PCP prescribed amitriptyline at 10 mg nightly. The PCP
told the patient to escalate the dosage by 10 mg every three to four days until the pain was
relieved, but not to exceed 50 mgs without consulting him.
When creating the prescription, the PCP intended to check off the 10-mg box in the
computerized physician order entry (CPOE), but inadvertently checked the 100-mg box, which
was right above it. In the medication instructions section, he indicated that five pills could be
taken per night, so the patient would not have to return to the pharmacy and pay additional co-
pay if he ultimately needed the larger dose.

The pharmacist had noticed that the dose seemed high and requested that a call be made to the
PCP prior to it being dispensed. A nurse at the PCP’s office picked up the call, and because she
was very busy that day, told the pharmacy to dispense the medication as it had been ordered —
she did not check the dose. Three days later, the patient took five of the 100-mg pills together.
Early the next morning, the PCP was contacted by an emergency department (ED) physician
who reported that the patient was in the ED reporting dizziness, an altered state of
consciousness, an inability to coordinate his movements and a rapid heartbeat.
He was further informed by the ED physician that the patient had taken five 100-mg
amitriptyline tablets. The PCP then checked the patient’s record and realized his mistake.
How could the design of the system be changed to prevent such errors in future?

Activity Title: Managing Clients using an Electronic Medical Record
Instructions:
1. This activity will be done in pairs. Look for a partner whom you can gather information from
and proceed with the typical health assessment, formulation of nursing diagnosis, nursing care
plan/s, implementation and evaluation strategies.

3. First, register as your interviewee as the patient.
4. Fill in relevant information on the patient dashboard as applicable, including: Diagnoses,
Latest Observations, Health Trend Summary, Weight Graph, Vitals, Appointments, Recent
Visits, Family Allergies
5. To do this, start a visit. Make sure to select Admit to Inpatient.
6. Upon changing status to Inpatient, make sure to capture vitals.
7. Once done, click on Visit Note and complete information in this section. Select a diagnosis
and provide under the clinical note all other information support the diagnosis. Note that the
diagnosis in this database follows the medical diagnosis, of which your nursing diagnosis can
match. Your nursing diagnosis, however, will be written under the clinical note. (e.g. A Primary

Make sure your visit note contains all relevant nursing care plans, including implementation
and evaluation strategies.
8. After completing all necessary entries, demonstrate ability to search information in patient’s
chart.
9. Go back to home and Fill in any known allergies.
10. Explain briefly all relevant information encoded and its relevance to the nursing care plan.

1. Passing rate for all examinations is 75% of the total number of items given
