Relative to this, all examinees of Davao City are required to bring the following:
1. Notice of Admission
2. Two (2) or more pencils (No. 2)
3. One (1) piece long size brown envelope
4. One (1) piece long transparent/plastic envelope (for keeping your valuables and
other allowed items)
5. Non-programmable calculators
6. For pregnant examinee, submit clearance from the OB-Gynecologists
❖Clearance should indicate that the examinee is cleared to take the licensure
examination.
❖No OB-Gyne Clearance, No Exam.
7. Snacks and lunch packed in transparent containers.
❖Food deliveries are not allowed.
❖Schools, review center, co-examinees, or any other groups are not allowed to
distribute food or drinks outside or inside the testing center premises, even
during break time.

IMPORTANT REMINDERS
1. Examinees are prohibited to use cellular phone, ear plugs, bluetooth, and other
electronic gadgets/devices. Examinees will be required to TURN OFF their
mobile phone/s before entering the examination venue. Phone/s must be
placed inside their bags and can only be used again upon exiting the venue;
2. Books, notes, review materials, and other printed materials containing coded
data/information/formula are not allowed inside the testing center premises.
Bringing of the said items inside the examination venue will be considered as
CHEATING or an ACT OF DISHONESTY. If an examinee is found guilty, his/her
examination papers shall be cancelled and shall either be suspended or
debarred from taking future examinations.
3. Avoid using Plastic PET Bottles, plastic wrappers and food packages. This is in
observance with the eco-solid waste initiatives of our partner schools.
4. Personal belongings brought by an examinee shall be for his/her own
account. The PRC will not be answerable for any such item or personal
belonging that might be lost during the examination without fault or
negligence on its part.
