 Check that all Full Body Harnesses and lanyards are in


good condition and free from defects.
 Is the area supervisor / Foreman competent to oversee
the work activity? With a minimum of 5 years’
experience.
4 Competency
Requirements  Is the Permit Holder (PH) aware of the risk associated
with the work activity.

 Does the work require review by UJV scaffold/rigging


Subject Matter Expert?

 Is there any SIMOPS issue at the work location i.e. RT,


Road closure, Pressure Testing, Energization, scaffolding
work or any other activity that could pose a risk?
 Are people working directly above or below task?
 Was the safety flyer communicated as minimum 24 hours
before commencing the job?
 Area Barricades & Signs in place to prevent unauthorized
personnel from entering into work area?
