Step 1: Review each “Hazard” with identified safety “Controls” and determine RAC (See above)

“Probability” is the likelihood to cause an incident, near miss, or accident and


identified as: Frequent, Likely, Occasional, Seldom or Unlikely. RAC Chart
“Severity” is the outcome/degree if an incident, near miss, or accident did E = Extremely High Risk
occur and identified as: Catastrophic, Critical, Marginal, or Negligible H = High Risk
Step 2: Identify the RAC (Probability/Severity) as E, H, M, or L for each M = Moderate Risk
“Hazard” on AHA. Annotate the overall highest RAC at the top of AHA. L = Low Risk
Job Steps Hazards Controls RAC
Manual clearing:-
 Insecticide bite  Usages of Gumboot & Hand gloves for protection against Injury on leg &

must be worn to avoid snake bite.
Unfit Equipment-
 Run over/Toppled  EHS Engg and P&M Engg. Check the fitness of equipment with
Unauthorized operator- respect to safety & mechanical aspects before employment of

Underground Utilities-  Inspect operator's certificate & license before engage in work.
 Damage of utilities or
Electrocution  Get then excavation clearance from concern dept & Work permit
Leveling by Man & Machinery Overhead lines- system to be followed.( Expose the utilities manually with power off / L
 Electrocution insulated materials before giving clearance)

Less Illumination:-
 Slips, Trips & Fall of Person  Restricting the vehicle movement by fencing.

 Provide sufficient lighting. Alteast 150 Lux.


Job Steps Hazards Controls RAC
Damage to underground electrical, Telecom
cables and water /Gas services:-  Follow Excavation clearance permit system. Look for cable route
 Electricity, fire & explosion. markers
 Make trail pits incase of doubt.
 Look for warning tapes/ cable covering mats/ concrete saddles/
sand padding.
 Adhere strictly to manual excavation in case of presence of
underground cables

Vehicle & Person Movement near the edge of


excavation:-  Provide hard barication with min clearance of 1.5m from the
 Fall of machinery/Person into the pit. excavated pit.
 Ensure adequate illumination, & danger lighting.
 Post reflective warning signs & blinkering lights to highlight deep
excavation.
Fall of objects (stone, boulder, soil etc.) into

 Provide adequate emergency access so that workmen can easily
come out of excavation Pit. Allow only minimum required number of
persons to work at the same time.

 Nose mask given to all nearby.
