Dear Ms. Areniego:

Re: Notice to explain in writing

This is to inform you that our office received an incident report allegedly last March 7,2021
based on the printed Collection list for the period of March 7,2021 you again issued 2 different
amounts to 1 serial number : PDC 13910 -white copy amounting to P 41,350.00 ( handwritten)
and PDC 13910-Pink copy amounting to 3,174.09 ( computerized) both PDC were not cancelled
in the system.

You are hereby given a period of FIVE (5) DAYS from receipt of this letter to submit your written
Explanation to answer why you should not be reprimanded suspended or terminated due to
the above-mentioned incident under Code of Discipline of the company under Article VI .Acts of
Disobedience Section6.4. Refusal to obey legitimate orders that are company related( S5D,DM)
6.5 Refusal to perform assigned tasks covered by job description or tasks related incidental or
necessary for the performance of employee’s job (S7d,S10D,DM)
Article IX. Negligence ,omission or carelessness Section 9.0 Negligence in the performance of
the employees official functions. Negligence under this section covers both simple and gross
( Simple S10d, S15d,DM Gross -DM)

Disciplinary action will depend on the seriousness of the offense or violation and its adverse
effect on the Company’s interests/reputation. Your failure to submit a written answer within the
period given will oblige the company to resolve the matter based on the information received.
