THIS CHANGE OF NAME DEED is made this 21/07/2024
BY me the undersigned Stuart Vellis-Humpheryes of 57 Edgar Road, Canterbury, Kent, UK,
CT1 1NS now or lately known as Stuart Humpheryes
IT IS WITNESSED AND HEREBY DECLARED as follows:
1. I absolutely fully and entirely renounce relinquish and abandon the use of my said
former name of Stuart Humpheryes and assume adopt and determine to take and
use from the date hereof the name of Stuart Vellis-Humpheryes in substitution
entirely for my former name of Stuart Humpheryes.
2. I undertake that I shall at all times hereafter in all records paperwork deeds
documents and other writings and in all actions and proceedings as well as in all
dealings (formal or otherwise) and transactions and on all occasions whatsoever use
and subscribe the said name of Stuart Vellis-Humpheryes as my name in substitution
for my former name of Stuart Humpheryes so relinquished as aforesaid to the intent
that I may hereafter be called known or distinguished not by the former name of
Stuart Humpheryes but by Stuart Vellis-Humpheryes only.
3. I authorise and require all persons at all times to designate describe and address me
by the adopted name of Stuart Vellis-Humpheryes.

Vellis-Humpheryes and also my said former name of Stuart Humpheryes and have affixed my
seal the day and year first above written.
