Community Service Project

Important Guidelines:

1. Each group must consist of at least 3 people, and maximum 5. Each group
selects one member to be a focal person who will be contacted by mentors for
updates. Each group selects a project idea that falls under one of the themes
discussed previously in the session.

The deadline to submit your selected theme/idea, names of chosen group


members, and group focal person is Tuesday, July 16, 2024.

2. The deadline to submit the final project report is Friday, July 26, 2024.

3. Please note that you need to ask for permission before taking any photos or
videos of participants. If they were minors, then you would need their parents'
consent.

*Please note that any failure to commit to the deadlines will directly affect your evaluation.

1. Project Description

● Objective:

Briefly describe the main objectives of the project. When you mention specific project
goals, make sure they are SMART goals.

● Rationale:

Summarize findings from any needs assessments conducted. What are the urgent needs
that you find in your community? What need/s will your project address? If needed,
conduct interviews/meetings with community members to gather more input about the
community’s needs. Furthermore, describe the current situation or context of the need
that you will work on before the project starts. State the expectations of your project.

● Beneficiaries:

Mention who will be your target participants, and describe the selection process.

● Description of Activities:

Describe the activities/steps planned to achieve the objective mentioned before. Briefly
mention the expected outcomes or changes as a result of the project.

2. Stakeholders:

- Involve volunteers through community boards, social media, and local organizations.
- Assign roles based on volunteers' skills and interests.
- Establish partnerships with local schools and organizations.
- Obtain necessary permissions and support.

3. Resources

● Resources Needed:
List the resources needed for the project including materials. Describe how the resources
were used. Were they sufficient and appropriately utilized?

4. Project Outcomes

● Achieved Objectives:
Describe to what extent the project’s objective was achieved. Describe the actual
outcomes or changes as a result of the project. Provide data and evidence where possible.

5. Lessons Learned

● Challenges:
List and describe any challenges faced during the project implementation.
● Successes (optional):
Highlight any successes or positive outcomes not initially anticipated (ex: success stories)
● Recommendations:
Provide recommendations for future projects based on lessons learned.
