AFFIDAVIT OF DISCREPANCY

I, ELVIRA B. PENALES, Filipino, of legal age, single, and a resident of Purok 5,


Mayacabac, Dauis, Bohol, Philippines, after being duly sworn in accordance with law, depose
and state:

1. That the first name of my father appearing in my CENOMAR is “RODULFO”;

2. That the person with first name: (1) “RODULFO” appearing in my CENOMAR, and the
person with first name (2) “RODOLFO” appearing in my Certificate of Live Birth, refer
to one and the same person and that the true and correct first name of my father is
“RODOLFO”;

3. That the first name “RODULFO” as reported in my CENOMAR as my father’s first name
is merely caused by a typographical error;

4. That I am executing this Affidavit to attest to the truth of the foregoing facts, for the
correction of my father’s first name in my CENOMAR, to clear any discrepancy in my
records with the respective government agencies and to support the Registration of
Marriage Certificate in the Local Civil Registrar and for whatever legal purpose that this
Affidavit may serve.
