COMMON ERRORS

FLIGHT PHASE COMMON ERRORS AFFECTED

COMPETENCIES
ENGINE START  SOP’s: Incorrect start procedure applied. Incorrect SOP flows. APK
 Unaware of starter limitations.
 Communication: Not informing the engineer to disconnect &
visually confirming pin removed and hand signal.
 SET not applied for (FUEL EFFICIENCY)
TAXI  Incorrect speeds during taxi. APK
 Max thrust limits not observed.(N1..40%) COM
 Communication: Missed calls, misinterpretation of instructions, or SAW
incorrect read-back to
 ATC, wrong clearance e.g taxiway, gate or Runway , Runway
incursions, hot spots notObserved. Last minute Change in
Intersection Take off- Revised speeds not calculated
TAKE OFF  Handling - Incorrect take off technique . Incorrect rotation FPM
technique. APK
 Landing gear not retracted and VFE Exceedance SAW
 Situational Awareness, Environment, Terrain etc
WINDSHEAR  Incorrect procedure: Delay reactions to the event. (Late selection APK
of TOGA.) Not complying with FPM
 SRS orders. Changing configuration during wind shear.
 Not monitoring pitch and flight path
 Situational Awareness : Weather - Not monitoring ATC, Pilot
reports .VFE Exceedance.
 If advised about Windshear Prior to Departure - TOGA not used /
Speeds not revised.
TCAS  Incorrect application of procedure APK
 Not maintaining the green band. Incorrect recovery procedure after FPM
“Clear of conflict”
 Communication incorrect and delayed. Filling of RA report not
communicated to ATC

 Incorrect application of procedure

TCAS AP/FD
 Not maintaining the green band. Incorrect recovery procedure after
“Clear of conflict”
 Communication incorrect and delayed. NOTE: If the simulator is
equipped with the AP/ FD TCAS function use this mode.
TAKE OFF  Loss of directional control and improper speed control. Incorrect FPM
EFATO rotation. Incorrect identification of failed engine. Delay in APK
retracting L/G which impacts the second segment climb gradient. SAW
 Flight path not stablised before ECAM actions (400 ft). Reverser
unlock ECAM to be applied.
 ECAM actions not stopped at engine secure. Not monitoring thrust
limitation. Task sharing for abnormal operation not observed.
 Engine out SID due terrain.
 Communication-Not declaring PAN / MAYDAY call, late call to
ATC if not following the SID. No confirmation of irreversible
actions. Pressing the EOCLR key on the MCDU, is an irreversible
action that leads to the loss of OEI computations.
OEI NPA  Incorrect /late approach management and Configuration, Straight APK
LOC in approach with OEI checklist not applied. FPA
 Automation modes incorrectly selected. LOC approach TRK/FPA
not used.LOC pb not selected.Altitude vs distance cross check not
carried out
OEI  Late reaction to the minimum call and continuing to descend. APK
GO-AROUND Improper or inadequate rudder inputs leading to a loss of heading. FPM
 Incorrect rotation.
 Late call for Flaps & L/G.
 FMA not reviewed
 Not informing ATC
OEI  Incorrect Positioning on Downwind (2.5 Nms), FCU Range/Mode APK
VISUAL incorrect Selections. FPM
 Incorrect Configurations
 Centering beta target during the approach.
 Incorrect ABEAM and time out not corrected for WIND.
 Resetting the rudder trim and maintaining rudder pedal
deflection, to prevent yawing effect.
 Incorrect flare technique.
 Incorrect use of Reverse thrust Unable to maintain directional
control after touchdown.
RAW DATA ILS  Not using the bird, over correcting, making large corrections, poor FPM
instrument scan,
 fixation on one instrument.
 Choosing a decelerated approach.
 Weather, ATC
CAT II / III  Incorrect seat position. Incorrect task sharing. Downgrading APK
procedure for the approach not complied with. Incorrect or no KNO
recognition for the reason of the activation of the Auto
 Land light.
 Incorrect understanding of the alert height.
 Incorrect application of RVR to determine landing category.
 Standard callouts not given.
LVTO  Not confirming position on the correct runway. Visual 90 m APK
requirement from threshold.
 Not using TOGA
ENGINE HIGH  Identification not established quickly APK
VIBRATIONS  Delay in resolving the cause of Vibrations PSD
 Not Complying with Operational Recommendations of Tech
Notice
 Not Completing VRS form for maintenance
STALL  Incorrect application of memory item. Delay in recognition of the FPM
stall.
 During application of thrust not counteracting pitch up movement
and activating the stall warning.
OEI VOR  Incorrect /late approach management and Configuration, Straight APK
in approach with OEI FPA
 checklist not applied.
 Automation modes incorrectly selected. Incorrect selection of
AP/FD mode
 Altitude vs distance cross check not carried out.
OVERWEIGHT  LANDING DISTANCE not calculated APK
LANDING  Overweight Landing C/L NOT reviewed FPM
 At Main Landing Gear Touchdown – Max reversers not Used
INCAPACITATION  02 calls not given to ascertain pilot incapacitation APK
 No STOP call given
