As Assistant CEO, I hereby charge and assign you to assist me in the performance of my
duties in terms and by authority of Section 16.2 of the Occupational Health and Safety Act,
85 of 1993.
You are to ensure the requirements of Environmental Regulations for Workplace
9(2) are completed within your area of responsibility.

In terms of the appointment stipulated above, you are required to be ensure the
following responsibilities are tended to:
1. You are to ensure that all firefighting equipment within your area of responsibility is in
good working condition.
2. Ensure that all employees are aware of your appointment as the fire marshal.
3. When a fire occurs in your area, sound the emergency alarm; then take action against
the fire, using the fire preventative equipment provided.
4. Ensure that you report to the Emergency Co-ordinator when the emergency
alarm is sounded and follow instructions given by the Emergency Co-ordinator.
5. You are to lead by example and always instil health and safety compliance in
the workplace.
6. You are to immediately report all incidents and accidents to the Manager.
7. Inform the Occupational Health and Safety (OHS) Officer on all OHS matters relating to
the above site when so requested.
8. This appointment is valid from the date of acceptance till the completion of the stipulated
construction work or until you are transferred to another site or until your appointment
with Western Cape Government (WCG) or any department within WCG is seized.
You are required to acknowledge your confirmation of training and your acceptance of the
responsibilities and implications of this appointment. Please initial each statement if you
