Prosecutor: Have you come up with findings and conclusions as to the result of your
examination, Sir/Ma’am?
Expert Witness: Yes, Sir/Ma’am. I have come up with a report on that.
Prosecutor: I'm showing you the question document record number QD 00_. What relation
has this report with the report you made?
Expert Witness: Sir/Ma’am, that is one and the same document that I have created.
Prosecutor: On the last portion of this document, there appears a signature on the top of
the typed written name, _________. What signature is this?
Expert Witness: Sir/Ma’am, that is my signature.
