Pursuant to the Labor Commissioner’s Nov. 27, 2019 Advisory Opinion, waivers are not required in
those crafts/types of work where no recognized apprenticeship program exists in the region where
the public work is located. Contractor is responsible for verifying whether recognized
apprenticeship programs exist in the region for each craft/type of work to be performed.
*Contractor is responsible for ensuring all crafts/types of work to be performed on the public work
are accounted for in this checklist. Attach additional pages if needed.

I affirm I am fully authorized to acknowledge, on behalf of the Contractor listed above, the anticipated
workforce, and acknowledge that changes to the anticipated workforce which may have an impact on
compliance with the Nevada Apprenticeship Utilization Act, 2019 will require the submittal of a revised form
within ten (10) working days of such change.
