Date : 06/12/2022

On Tuesday 06-12-2022 an incident occupied on the Case-parker Machine. Mr Amoo Ayokunmi was the
operator on the machine. During my investigation I was informed that he got stock into the machine
while trying to adjust a carton on the conveyor.

I have asked him to write a report on what happened, which he has wrote and it has been attach to this
report.

In his report and my investigation I can only come to a conclusion that he needs more training on the
Job, and he should understand that safety is key in our process.

Sir, I hereby suggest that a safety training on how to run all machines should be organize for him and the
order operators on the shop floor to prevent reoccurrence of such act in future.
