Instruction - Patient Navigation
1. Upon receiving a call or a referral, ask the concern of the caller and what service they
need to avail. FILL OUT the form which is seen below. Be specific regarding the
informant (if self or another person), the full name of the patient, what CASE (specific
department) it belongs to, and don’t forget to include FULL address (include Barangay,
Municipality, and Province) along with all the other information given in the form. Also
include the TIME of call on the upper right corner and the time ended lower left plus your
signature and Ma’am Keith (Navi-OIC).

2. After filling out the forms and attending to the concern of the patient, the next thing to do
is to compile it for CENSUS. Due to patient data privacy, only Ma’am Keith’s laptop has
access on the site seen in Figure 2 wherein you will see the links for the census which is
usually “Other Hospital/Self-Referral” form. The interface of the site is seen in the
figure below (Figure 2).

3. After opening the link, you will see a GOOGLE FORM as seen in Figure 3 in which all
the details are needed to be filled out by the Email used by the Navi which is
[email protected] as seen below. Fill out the information being asked by the google
form then just click “Submit”. The information to be filled out is the information entered in
Figure 1 which is the basic information of the patient. You also need to classify the case
based on the level of care which can be seen below:

4. After submitting an entry, you can check the patient responses on Ma’am Keith’s laptop
entitled Patient Navigation (Responses) as seen below (Figure 4). The information
there would include the ones you filled out on the Google form at Figure 3. Next is you
need to manually enter the DISPOSITION of the patient wherein you will categorize the
patient’s navigation outcome. For example as seen below, those were for follow-ups with
indicated Entry Point which are their corresponding departments for their services.

5. YOU’RE DONE! The data is already for safe keeping. However, there are still other
census that needs to be done as well which is the CONSOLIDATE which is entitled
Performance Indicator Daily Tracker wherein you will base the numbers on the total
census that day combined with the census of the NAVI/OPCEN and EMT-EOC which is
seen in the image below.

NOTE: The daily trackers should be updated every 12 hours with their corresponding
spreadsheets (7AM to 7PM, 7PM to 7AM). Performance Indicator Daily Tracker (Figure 5)
should also be PRINTED every 12-hour shift (7PM and 7AM) DAILY to be checked by Ma’am
Keith.
