Saipem Classification - General Use


Definition
 A formal written system used to control certain types of work that are potential hazardous;
it is also a means of communication between site management and supervisors and those
who carry out the work
 A document which specifies the work to be done and the precautions to be taken allowing
good monitoring of potentially conflicting activities

in order to better identify & manage their related
risks

Attention: Your signature on a PTW imposes certain legal duties on you,


including your personal accountability in case of an accident (if you did
not fulfill adequately your duties)

Why is it necessary?
 It is required as per owner’s Procedures
 It reduces the risks
 It promotes coordination
 It allows communication and awareness among the Site Management, Foremen/Supervisors
and all personnel who carry out the operations

HSE statistics show how 1/3 of all accidents is due to


lack of/deficiency in PTW:
 Companies are not checking PTW adequately Procedures
 Permits are not adequately identifying potential hazards !!?!!
 Permits are unclear on needed control measures
 Companies are not applying for PTW at all

When it is necessary?

There is NO single list of tasks that require PTW!


 Whenever required by Risk Assessment and/or PTW Project procedure
 When an activity that requires to lower the safety modification measures normally in place is
performed (usually during maintenance)
 Whenever it is required to manage simultaneous operations that might clash one with the
other
 Whenever has to be performed a NOT routine operation

The Objectives and Functions of PTW system are
 To ensure that proper authorization is given to carry out work at a certain
time and place.

 To ensure that personnel who carry out the work clearly understand the
nature of the work, the hazards of the job, and the limitations on the work
and the time.

 To provide a record showing that the type of work being carried out and
Procedures have been checked by an approved person.

 To provide a procedure for times when work must be suspended.

 To provide for the procedures or arrangements for other activities that may
interact or effect one another.

 To ensure that the supervisors are fully aware of the work being done.

 To specify the precautions to be taken before starting, during and after the
work is completed.

Radiography Work Permit
The Radiography Work Permit is to control all site
radiography and the use of ionising Radiation, it is
imperative that the controls remain in compliance with
government legislation regulating the movement and
uses of radio active sources; the Permit should be used
in conjunction and in agreement made between the
Contractor FHSE Coord, Contractor Permit Coord, and
the subcontractor RPS; prior to the signing of the
Permit to Work the subcontractor shall demonstrate
that the area is completely fenced by means of warning
tapes and provide appropriate signboards and that
there is no risk to personnel from the use of radioactive
substances

Entry certificate for Confined Spaces
The Confined Space Certificate, shall be used in conjunction with
the Permit to Work (Cold, Hot and Radiography); entry into
confined spaces is subject to special requirements being put into
place prior to the commencement of work especially gas test is
mandatory to carry out;
This will include but not be limited to:

 Condition monitoring
 Presence of a Safety Watch Person
 PPE/RPE
 Emergency Escape Plan

Electrical Work Certificate
This Certificate shall be used in conjunction with the Permit to Work
(both Cold and Hot), for any activity to be carried out on live electrical
equipment or plant; the Electrical Competent Person should complete
this form and demonstrate that all necessary isolations have been
made and that the system or plant is safe to be worked on; the
electrical work certificate shall be utilized when an energization or de-
energization operation should be carried out; the Electrical Work
Certificate shall be issued as a minimum, but not exhaustively for
following activities:
 Any activity on an live electrical equipment
 Connection to the electrical supply
 Inside areas electrically classified
 To carry out any Isolation/lock-out/tag-out

• Adequate shoring or slopping is required
depending on the depth.
• If any pipes or cables are discovered, the job has

LIFTING REQUIREMENTS
•Lifting Certificate and Genera l Lifting Requirements
•Critical Crane Lifts- Critical Lifting Plan {SA-9644 ).
I. Lifting Certificate and General Lifting Requirements
• Starting April 1, 2017, Lifting Certificate will NOT be required
on all General Lifting activities (deemed not critical).
All permits associated with Lifting Activities will require
Hot Work Permit along with relative attachments {MS , JSA, Etc.)
which will also contain relevant information;
e.g. associated weight bracket and rigger requirements should be
mentioned (Ref: GI.07.028.2009- Par. 8.0):
-Rigger Ill - Up to 10 tons (except lifting activities deemed critical)

-Rigger II - Above 10 tons but less than 40 tons

-Rigger I- All loads and Critical Lifts.

II. Critical Lifting Plan (SA-9644)

• SA-9644 Critical Lifting Plan will only be attached to all


Critical Lifting Work Permits along with all relevant
attachments for the lift,
• e.g. drawings, third party certificates and inspections, load
charts, etc.

• Critical lifts at site: (Ref:Gl.07.028.2009-Par. 5.0)


-Tandem lifts.
-Tailing/Tandem (multiple) or turning lifts.
-Heavy Lift (Lifting loads 40 tons and above).
-Crane lifts more than 85% of the cranes capacity.
-Crane lifts that requires attachment to the main boom e.g.
Fly jib.

Job Safety Analysis (JSA): is a simple mean of looking at the


work task and considering what is the safest way to
complete it.
Three basic stages in conducting a JSA are:

- Responsible for supervising the work group carrying out the job.

- Ensure the work group carrying out the work is fully briefed have clearly
understood instructions given, including the scope of their activity and
the relevant risks.
- Remain at the worksite until the completion of the task.

Roles and Responsibilities
CONTRACTOR CONSTRUCTION MANAGER:

• The Construction Manager shall ensure that all work is carried out under
the controls of Permit to Work System.

• Nominate a Contractor Permit Coordinator to ensure that the


coordination of permits is administered correctly;

• Ensure the proper coordination of construction activities between


Contractor and subcontractors;

• Verify that the subcontractor is providing the correct information and


equipment for the scope of work;

• Ensure training is given to all persons requiring to use a Permit to Work;

• Nominate personnel and authorize them to perform the duties of permit


to Work Signatories;

Contractor Issuing Authority
The Issuing Authority is responsible for carrying out all preliminary checks required and
will sign the Permit to Work to verify that all requirements have been met and that the
work area is in a safe condition for the work to commence.

• Ensures that the Receiving Authority understands what, where, and how the work is to
be performed to ensure compliance with the conditions stipulated on the permit;

• To ensure that the Receiving Authority supervises the work group and is satisfied that
the persons carrying out the work use the proper tools and equipment for the job that
the Performing Authority informs the work group of the safe working practices;

• Inspect the worksite periodically to ensure there is no change in condition;

• Suspend the work immediately if there is any breach of this procedure or change in
conditions and to notify PTW Coordinator for the reason;
• Ensure a good standard of housekeeping is maintained throughout the work and
that all equipment is removed upon completion;

• Inform of any change in the working conditions of the surrounding areas in the
vicinity of the working area.

Contractor Receiving Authority

The contractor Receiving Authority is responsible to:

Ensure the work group carrying out the work is fully briefed usually by
means of dedicated toolbox talk;

Ensure that they have clearly understood instructions given, including the
scope of their activity and the relevant risks;

Remain at the worksite until the completion of the task. After signing the
Permit to Work Receiving Authority shall ensure that all copies of the Permit
to Work are correctly distributed and that a copy is retained and posted at the
work site. In addition the contractor Receiving Authority shall:

Use proper tools and equipment for the safe execution of the job; and
ensuring the worksite is left in a safe and tidy condition.

Authorized Gas Tester (AGT)
 The Authorized Gas Tester shall assign to verify the testing has been done.
 Gas tester shall be formally trained, certified by SA approved third party and
competent in carrying out the gas test.
 A list of Authorized Gas testers shall be submitted to the Contractor Field HSE
Manager and Contractor Permit Coordinator

Fire Watchers
Fire Watchers
Personnel appointed as Fire Watchers shall be adequately trained and
shall

monitor work areas, and the precautionary measures taken in those


areas,

when specified by the Issuing Authority on the PTW. Fire Watchers shall

undertake fire response activities as required

In case a fire started by the hot work, the fire watch shall:
• Stop the work
• Sound the portable air horn/shout fire fire…….
• Notify to the Performing authority,
and attempt to put out the fire if it’s safe to do so.

Confined Space Attendant

 The Confined Space Attendant will remain outside the confined space, in a safe
atmosphere, and perform the assigned duties under this procedure, including:

 Use of tally board to record the names, entry and exit times for personnel
entering or leaving the confined space

 Monitoring conditions and activities inside and outside the confined space to
determine if it is safe for entrants.

 Remain outside the confined space during entry operations until relieved by
another attendant.

 Maintain effective and continuous communication with authorized entrants


during entry.

In the event an emergency the Confined Space Attendant


must initiate an emergency response,
he should:
• Sound the air horn and/or make radio contact
• Identify himself
• State exact location
• Explain the type of emergency
• Identify type of assistance needed

The Confined Space Attendant must never


enter a confined space to assist an entrant.

Suspension/Cancel/Invalid Permits
The Permit to Work can be invalidated under any of the following conditions:
• Two hours or more pass between the preliminary tests and the start of the
work (the maximum acceptable delay shall be anyway evaluated based on
the specific conditions);
• For gas testing in confined spaces, the maximum time between the gas
test and the entry / work commencement exceeds 30 minutes (the
maximum acceptable delay shall be anyway evaluated based on the
specific conditions);
• The conditions on the permit are not complied with;
• Area or unit conditions change, e.g. venting or spillages of hazardous
materials occurred, etc.;
• Plant fire & gas / emergency alarm is released;

Permit Issuance

• Any person requiring permit to work must submit their request to


Contractor Permit Coordinator at least one working day prior to the
commencement of the work, 23 meters away from the Aramco areas.
• Prior to issuing the permit, the Contractor Permit Coordinator should satisfy
himself that the safety conditions described in the permit are correctly
applied.
• If any conditions required by the work permit cannot be met, the permit is
to be withdrawn by the Contractor Construction Manager or the Area
Superintendent and the work must not be allowed, until remedial actions
have been taken.

Conclusion
REMEMBER
 The issuance of Work Permit does not by itself make a job safe.
SAFETY can only be achieved by those preparing the work & those
carrying out the work.

 If you are unsure of any instruction on a Work Permit or in any


doubt regarding procedure, STOP & ask your immediate Supervisor
for guidance.
